Join our Urgent Community Response (UCR) Team, a dynamic and highly skilled multidisciplinary team providing rapid assessment and intervention across Barking & Dagenham, Havering and Redbridge. Our service responds to physical health crises in patients' own homes, helping people avoid unnecessary hospital admissions and supporting them to remain independent within their communities.

This is an exciting opportunity for an enthusiastic and motivated Occupational Therapist who enjoys autonomous working, complex clinical decision-making and collaborative multidisciplinary practice.

Main duties of the job As part of our UCR team, you will undertake urgent assessments within patients' homes, delivering timely interventions that support recovery, reduce hospital admissions and promote independence.

You will need strong clinical reasoning, excellent communication skills and the confidence to make decisions in often complex and rapidly changing situations. No two days are the same, and you'll be supported by an experienced MDT that values teamwork, innovation and shared learning.

We are particularly interested in candidates who are passionate about frailty, urgent care and falls management. You will regularly assess patients who have experienced falls and may remain on the floor for extended periods, requiring a compassionate, holistic and proactive approach.

Job description Job responsibilities

Starting with NELFT NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop-in sessions focusing on engagement, health and wellbeing and key processes. The induction will be held at our head office in Rainham, Essex.

Probationary Period This post will be subject to a probationary period. Internal applicants are exempt from the probationary period (unless you are an internal applicant currently part way through a probationary period or currently a bank member of staff).

High Cost Area Supplement – Outer London (B&D, Redbridge, Havering and Waltham Forest, CEME) This post also attracts payment for High Cost Area Supplement of 15% of the basic salary (with a minimum of £4,714 to a maximum of £5,941 p.a. pro rata for part time).
