Job summary

Pharmacy technicians have an important role working alongside the Clinical Pharmacists in the PCN which supports the long term NHS plan of keeping patients well and out of hospital.

The PCN consists of 4 practices -: Maple Access Partnership, Mayfield Surgery, Favell Plus Surgery and The Mounts Medical Centre. This role will work across all 4 serving the local patient population.

Pharmacy technicians assist with medicines safety, use, repeat prescribing, medicines reconciliation and hospital discharge medication changes.

This role is perfect for an adaptable, organised professional who can work collaboratively to implement medicines optimisation and safety initiatives.

Main duties of the job

Medicines Reconciliation-: Undertake comprehensive medication reconciliation following hospital discharges, outpatient clinic letters, and transfers of care to ensure the safe and accurate continuation of treatment.Discharge Follow-ups-: Under the supervision of a Senior Clinical Pharmacist, facilitate the timely, safe, and accurate supply of prescribed medications following hospital discharge and other care transitions.Prescribing Safety-: Manage practice prescribing systems and coordinate medication safety monitoring processes, with a particular focus on high-risk medicines to support safe prescribing practices.Queries & Communication-: Act as a key point of contact for medicine-related queries, liaising with patients, practice clinicians, and local community pharmacies to resolve medication supply issues and identify suitable alternatives where required.Structured Medication Review (SMR) Support-: Assist Clinical Pharmacists in preparing for Structured Medication Reviews (SMRs) by gathering relevant clinical information, monitoring compliance, and supporting medication optimisation activities.Clinical Audits & Quality Improvement-: Participate in prescribing audits and quality improvement initiatives to promote safe, evidence-based, and cost-effective prescribing in line with local and national guidelines.

Work with primary care team and patients to resolve medication queries.

Supporting medication reviews and medication reconciliation

Assisting with repeat prescribing and ensuring safe dispensing.

Monitoring adherence to practice, local and national prescribing policies and guidance.

Supporting clinical pharmacists in delivering clinical pharmacy services.

Providing advice and information to patients and other healthcare professionals.

Undertake structured medication reviews.

Triaging and managing common ailments.

Conducting medication audits.

Providing information to patients on medicines and treatment.

Supporting patients with medication changes and managing their medication.

Signposting patients to relevant support services.

Prescribing Audits in line with CQC and QOF targets

Supporting the implementation of safe and efficient repeat prescribing systems.

Person Specification

Qualifications Essential

BTECH/NVQ 3 in pharmacy services. Essential Experience of working as a qualified, registered pharmacy technician.
