Job summary
We are looking for an enthusiastic and motivated Band 6 Physiotherapist to join our dynamic Responsive Services Team. This is an exciting opportunity to work within a fast-paced, multidisciplinary environment, delivering urgent assessment, intervention, and rehabilitation to patients in the community.
Our Responsive Services aim to prevent unnecessary hospital admissions, facilitate early discharge, and support patients to remain safely at home. You will work closely with a wide range of professionals, including occupational therapists, nurses, paramedics, rehabilitation assistants and social care colleagues.
Responsive Services is made up from Intermediate Care Team, Urgent Community Response, Virtual ward and the Referral Hub.
Key Responsibilities
Provide timely, high-quality physiotherapy assessment and intervention to patients in their own homes or community settings Manage a complex and varied caseload Support hospital avoidance and early supported discharge pathways Deliver rehabilitation programmes to improve mobility, independence, and quality of life Work collaboratively within a multidisciplinary team to develop person-centred care plans Provide clinical advice and guidance to junior staff, support workers, and students Participate in service development and quality improvement initiatives
Main duties of the job
We are seeking a physiotherapist who is:
HCPC registered with a degree in Physiotherapy Competent in managing acute presentations and complex patients Confident in autonomous decision-making and lone working Skilled in assessing mobility, falls risk, and functional ability A strong communicator with excellent interpersonal skills Flexible, proactive, and responsive to service demands
The service runs from 8am-8pm 7 days a week, 365 days a year, the post holder will be expected to work on a rota basis.
Due to the nature of the role it is essential applicants are able to drive, have a clean driving license and a car suitable for business purposes.

Job description Job responsibilities
Working within the multidisciplinary team the post holder will work under the guidance of the lead therapist within Responsive Services.
Responsive Services consists of the Intermediate Care Team (ICT), Urgent Community Response (UCR) team, Virtual Ward (VW) and Referral hub.
You will be responsible for assessing the health and social care needs of patients and their carers withing the community, aiming to prevent the need for patients to be admitted to hospital and facilitating their discharge home after an acute admission.
You will act as a role model for junior team members and contribute to the promotion of an environment which encourages and supports development of staff using evidence-based nursing and therapy practice.
You will be expected to utilise a variety of strategies to support the patient at home and to communicate effectively with family, carers, the multidisciplinary team, voluntary organisations, and other agencies, to facilitate rehabilitation and promote independence.
The Responsive Service teams function from 8am to 8pm Monday Sunday. The post holder will be expected to work shift patterns to cover service delivery.
