Job summary

We are looking for a dynamic, motivated and forward-thinking Registered Nurse who demonstrates a passion for supporting patients living with Parkinsons, their families and carers

This is an exciting opportunity for a Specialist Nurse to play a key role in delivering first rate care in the community setting.

The post holder will work as part of a Neuro Specialist Team and the wider multidisciplinary team (MDT), managing a Parkinsons caseload.

The post holder will need to have good organisational skills, with experience of complex decision making and excellent communication skills, ensuring that patients with Parkinsons Disease receive optimal care and live well with their condition.

They will need to hold an independent prescribing qualification or be willing to undertake this as an essential part of this role.

A full UK driving license and access to a car are also essential, as this role involves community based working.

This position is for 30 hours per week.

Main duties of the job

The successful candidate will be responsible for:

Managing an active caseload of people with Parkinsons Disease, throughout their disease pathway. Working with patients and their carers to provide interventions and to develop self-management plans to optimise health and quality of life and reduce avoidable hospital admission.Working in collaboration with patients and their carers, GPs & Hospital Consultants to achieve optimum therapeutic benefit from medication and treatment regimes.Supporting the multidisciplinary team to provide specialist care to patients with Parkinsons Disease and be an integral part of the Community Stroke and Neuro Rehabilitation Team and Neuro Specialist Team at First Community.Developing and delivering education and training relating to specialist area - for the MDT, for other health and social care professionals and for people affected by their condition and carers.

Person Specification

Other Essential

Demonstrates a passion for the care provision for those with Parkinsons Disease and other neurological conditions. Enthusiasm to learn and develop further expertise in this field. A full UK driving license and access to a car are also essential, as this role involves community based working.

Experience Essential

Minimum 5 years post registration experience with evidence of experience working in a relevant specialist field Evidence of successful service developments and innovations. Evidence of delivering teaching/training. Evidence of cross boundary working in partnership with other agencies. Experience of working with neurological or end of life patients. Up to date evidence of continued professional development. Evidence of knowledge and experience of proactive multi-disciplinary team working, being flexible and responding positively to change.

Desirable

Experience of Audit & Benchmarking Risk Management. Experience of line management of teams.

Knowledge Essential

Knowledge and understanding of Parkinsons Disease as a neurological condition. Knowledge and understanding current guidance/policy (e.g., NICE relevant best practice standards) Good working knowledge of Microsoft Teams and Microsoft Office including Outlook, Excel, Word and PowerPoint. Knowledge and understanding of CQC requirements.

Skills Essential

Qualifications Essential

NMC Registration (Degree Level) Evidence of further relevant post registration courses to Degree level. Independent Prescribing Qualification (or prepared to work toward obtaining one). MSC or willing to work towards.
