Job summary Bank Occupational Therapist - Band 6Join the Merton Domiciliary Therapy Team as a Bank Occupational Therapist, supporting adults with frailty and complex needs to remain independent at home. Working within a multidisciplinary team, you will assess patients and deliver personalised rehabilitation plans to promote recovery, mobility, and wellbeing. This flexible role involves providing home-based assessments, contributing to rehabilitation planning, and supporting the development of junior staff where appropriate. Your work will help prevent hospital admissions and support safe, effective care in the community. Main duties of the job

Deliver high-quality, person-centred rehabilitation to patients in their homes, care homes, and community settings as part of the MDT. Assess and provide tailored OT interventions for patients with complex physical, cognitive and functional needs. Manage a caseload of patients, working autonomously and collaborating with MDT colleagues to ensure coordinated care. Promote independence through rehabilitation, mobility support, equipment provision and environmental adaptations. Supervise and mentor junior staff, students and rehabilitation support workers, fostering a supportive learning environment. Lead quality improvement initiatives and contribute to service development, audits and clinical pathway improvements. Liaise with statutory, voluntary and community services to support integrated care delivery. Maintain accurate clinical records and participate in governance activities to ensure high standards of care. Support the professional development of the team through training and continuous learning.

Person Specification

Qualifications Essential

Degree or diploma in Occupational Therapy Current AHP with UK HCPC registration Up to date evidence & relevant knowledge through CPD as evidenced by portfolio Moving and Handling training

Desirable

Membership of Royal College of Occupational Therapy and / or relevant special interest groups Attendance at Clinical Educator Course

Experience Essential

Appropriate post-qualifying experience in a range of areas acute care of the elderly, neurology and community Appreciation of constraints to working in the community A clear understanding of current NHS issues, professional standards, CPD, clinical governance and experience of audit Ability to assess patient conditions, establish the patient's problem, formulate and implement a treatment plan Has sound knowledge of the Mental Health Act, lone working policies, manual handling polices and other relevant legislations and policies Experience of MDT working and understanding their roles Some experience of supervising juniors/assistant/students Some experience in research / literature reviews Experience in community work within rehab Post registration experience at band 5 level in a wide range of clinical areas including elderly rehabilitation, community rehabilitation, falls management, dementia care, respiratory care and orthopaedics Experience of line managing / Supervision of staff Some understanding/experience in quality improvement and service evaluation. Moving and handling assessments and report writing. Aware of risk-management and issues relating to working the community and people's home Using NHS clinical systems

Desirable

Experience in teaching and training Triage experience Positioning and contracture management
