We are seeking a professionally dedicated Chronic Disease nurse in the Practice and to contribute, as necessary, to the wider Practice Nursing Team effort in the support of patients. The role would be to join an existing nursing team of specialist nurses in asthma, copd and diabetes supported by a mdt team of paramedics, nurse practitioners and gps.
We can be flexible in the job requirements in order to accommodate to right candidate offering between 16 - 24 hours a week.

The following are the core responsibilities we are looking for:
a. To assist in chronic disease management of our Asthma, COPD & Diabetes patients including, where necessary, referring patients to secondary/specialist care.
b. To implement and evaluate individual treatment plans for chronic disease patients that promote health and well-being.
c. To identify, manage and support patients at risk of developing long term conditions, preventing adverse effects to patients health.
d. To contribute to the overall effort of the Practice nurse team
e. To request and process pathology services as necessary.
f. To maintain accurate clinical records in conjunction with extant legislation.
g. To liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients etc.)
h. To delegate clinical responsibilities appropriately.
i. To support the clinical team with all safeguarding matters.
j. To work effectively as a team and autonomously to ensure patient care is maintained and upheld in accordance with guidance, protocol and contracts.
About us
Bedford Street Surgery is a two site practice located in Bletchley, Milton Keynes with a branch site in Furzton, Milton Keynes. We currently have over 16300 patients across both sites and a supportive and care focused team.
We have a well established clinical MDT team consisting of paramedics, prescribing nurse practitioners, respiratory nurse, diabetes nurse, health care assistants, practice nurses, a pharmacist and gps who are supported by our management and admin team of secretaries, care navigators and care coordinators.
Bedford Street Surgery is part of the South West Network where we are able to work together to provide our patients with additional services and care enhancing the practice with first contact practitioners, Integrated Support mental health services, dietician, social prescriber and health and wellbeing coaches.
We offer monthly mdt meetings, safeguarding meetings and shared learning meetings to support patients and staff alike.

The successful candidate will be professional, motivated and able to work collaboratively and autonomously as part of our Practice multidisciplinary team.
On successful appointment you will ensure nursing services are delivered effectively to the entitled patient population for chronic illness management in line with guidelines, protocols and contractual agreements.
The role will entail a clinic for telephone and face to face appointments to assess, review and follow up in patient care. You would be required to undertake the review of pathology if required and liaise with internal ad external staff to ensure the patient receives the right care first time.
The successful candidate will, dependant on skills and experience, take on chronic illness management of the practice patients in the reviewing and delivery of clinical policy and procedure.

Qualifications Essential
Essential Registered Nurse (Nursing and Midwifery Council) Experience of chronic disease management (specifically with experience of the management of Asthma/COPD/Diabetes).
