Are you looking to join a dedicated and caring team who support local patients when they need it the most?

St Leonard's Hospice is recruiting a Hospice Practitioner to provide care for patients on our in-patient unit and support our community-facing Hospice@Home service. Based at the Hospice, our Hospice Practitioners work with the existing team of experienced Advanced Clinical Practitioners, Resident doctors and Consultants in Palliative Medicine to ensure patients receive responsive and high quality care.

This is a part-time position working 1 day per week (2 sessions) and a discussion can take place at interview with regards to which day of the week. There will also be a requirement to be part of the non-resident IPU on-call rota which is currently 1 in 6 nights which equates to a further 2 sessions per week.

We do not have sponsorship status and therefore all our offers are made conditional upon our candidates submitting evidence of their right to work in the UK in conjunction with our other pre-employment checks.

Main duties of the job

Along with the above, your main duties will be to:

Provide support and advice to community and hospital specialist palliative care teams and other professionalsActively participate in multi-disciplinary team discussions and meetingsSupport the development of clinical governance, including evidencing a commitment to clinical audit, risk management, staff development, within the Hospice governance framework

Our ideal candidates will:

Be a doctor with a minimum completion of the foundation years trainingHave excellent communication skills and be able to adapt their style to different audiencesBe able to work as part of a multi-disciplinary team and provide leadership and directionHave experience in clinical governance, education and advance care planning

Experience within palliative care would be an advantage however your overriding quality will be that you are committed to putting Hospice patients and their families at the centre of all clinical decision making.

Job description Job responsibilities

Our Hospice Practitioners provide care for patients on our in-patient unit and also support our community-facing Hospice@Home service. Based at the hospice, our Hospice Practitioners will work with the existing team of experienced Advanced Clinical Practitioners, Resident doctors and Consultants in Palliative Medicine to ensure patients receive responsive and high quality care.

Main duties and responsibilities

Professional

1. Provide medical input and clinical care as required to Hospice patients who are either receiving care on the IPU or in the community, alongside the multi-disciplinary team.

2. Provide support and advice to community and hospital specialist palliative care teams and other professionals.

3. Liaise with other healthcare professionals to plan and deliver treatment in keeping with the patients wishes.

4. Actively participate in multi-disciplinary team discussions and meetings.

5. Maintain and administer medical records as per Hospice policy.

6. Be involved in audit, research and quality improvement programmes as appropriate.

Managerial

1. Work closely with the Medical Director to ensure provision of medical input throughout the Hospice.

2. Support the development of clinical governance, including evidencing a commitment to clinical audit, risk management, staff development, within the Hospice governance framework.

3. Assist in delivering undergraduate teaching for medical student placements in the hospice.

Additional Information

1. The post-holder must participate in the on-call IPU rota. When on-call, the post-holder will have consultant cover as a second tier. On call is currently 1 in 6 and is non-resident provided they remain within a 45-minute drive from the premises.

2. Doctors provide prospective cover for each others leave. Additional weekday and on-call sessions worked to back-fill other forms of leave attracts sessional payment at the usual rate.

Person Specification

Qualifications Essential

Full registration with the GMC, with a licence to practice As a minimum, completion of foundation years training, or equivalent for doctors trained overseas

Desirable

Completion of Core Medical training (post-foundation) medical experience Higher level qualification in palliative medicine

Personal Essential

Ability to provide leadership and direction relating to colleagues Excellent team working and interpersonal skills Able to recognise and respect the skills of others within the whole Hospice team and effectively collaborate with colleagues both internally and external partnerships Ability to respond effectively and quickly to changing circumstances and situations Emotionally resilient and has an awareness of the need for good structures of support to cope with death and dying within the field of palliative medicine

Communication Essential

Able to demonstrate empathy towards patients and families, including ability to adapt style and practice to individual situations (i.e. patient-centred) Ability to communicate clearly - in speech and writing, and to maintain confidentiality

Desirable

Higher level accreditation in advanced communication skills

IT Essential

Computer literate able to use email and Microsoft programmes including Word and Outlook, and able to learn to use electronic patient records / information systems

Desirable

Experience in use of SystmOne (S1) and Core Patient Database (CPD) Experience of teleconferencing for patient care (e.g. MS Teams etc.)

Experience Essential

Knowledge of holistic care and the principles of palliative care, and ability to develop independent decision making in this specialist field Experience in clinical governance, education and advance care planning

Desirable

Experience in specialist palliative medicine

Other requirements Essential

Be prepared to assist with Hospice fundraising activities Ability to attend the Hospice within 45 minutes when required during on-call periods
