The West South Somerset neighbourhood are looking for a nursing colleague to join our integrated team, to deliver patient care in the community! Our team cover the geographical area across Chard, Crewkerne, Ilminster, South Petherton and surrounding areas. This post will be based with the Ilminster team.
The role of the Community Staff Nurse has changed over the years, and we are now looking after patients in their home environment including complex wound management, palliative care and end of life care, medicines management, as well as having a high standard of holistic assessment and care planning skills.
You will be working independently to support with managing the care of patients in our local community and wider Neighbourhood.
We are recruiting due to staff development and promotion - as a service we pride ourselves on colleague development and succession planning. We provide generous support for CPD internal & external opportunities, and for district nursing colleagues specifically we encourage you to apply for the specialist practice qualification which you would be supported to complete on either full or part-time basis.

Main duties of the job This post will involve working between the hours of 08:00 -- 22:00 including weekends and bank holidays on a rota basis.Shift patterns include: 8am-4pm, 9am-5pm, 10am-6pm, 2pm-10pm. We are open to hearing from applicants who are looking for either full or part-time hours on a permanent basis.In Somerset, we're working with health and care partners to care for patients at home safely, rather than being in hospital.This includes either preventing avoidable admissions into hospital or supporting early discharge out of hospital. The service can help patients to complete their treatment or be monitored prior to surgery from the comfort of their own home.

Person Specification
Qualifications Essential
Registered General Nurse Level one registration with appropriate NMC requirements
Desirable
Experience of working with multi agent services, and experience of working within a community setting.

Compassionate- exceptional interpersonal skills with the ability to communicate effectively with patients, carers and relatives remaining sensitive and empathetic Listens to others' views respecting and valuing individual patient needs Excellent organisational skills, ability to manage own time and plan timed activities for staff and patients.
Desirable
High standard of clinical work and competence to meet the needs of the service Knowledge of current tissue viability and pressure area management Experience in Care of Elderly Experience in Palliative Care IV administration competency
