Job summary

We are seeking a motivated and compassionate Salaried GP to join our friendly and family orientated practice for 4-6 sessions on a permanent contract; the post will cover two practices in the Sunderland North Primary Care Network (PCN) on a 4:2 ratio. Main duties of the job

The successful candidate will provide high-quality, patient-centred care within the multi-disciplinary team, through routine consultations, chronic disease management, preventative healthcare, and management of acute medical conditions. The role includes working collaboratively within a supportive multidisciplinary team, maintaining accurate clinical records, managing referrals and investigations, and contributing to the ongoing development and quality improvement of the practice.

Our practice team, managed by our Practice Manager, is comprised of the following:

2 x GP Partners

1 x Advanced Nurse Practitioner

2 x Practice Nurses

1 x Healthcare Assistant

Fully compliment of Administrative support

We also work alongside our Allied Healthcare Professionals under the Additional Roles Reimbursement Scheme. This team includes a GP, Pharmacists, Pharmacy Technicians, Mental Health Workers, MDT Coordinator, Social Prescribers and Health Coaches.

Job description Job responsibilities

The successful candidate will:

Provide high-quality, patient-centred care through face-to-face, telephone, and online consultations.

Assess, diagnose, treat, and manage acute and chronic medical conditions in line with current clinical guidelines and best practice.

Undertake routine health reviews, preventative care, and health promotion activities.

Prescribe medications safely and appropriately within practice and national prescribing policies.

Maintain accurate and contemporaneous patient records using the EMIS clinical system.

Work collaboratively with the multidisciplinary practice team, including nurses, pharmacists, and administrative staff.

Making professional, autonomous decisions in relation to presenting problems, whether self-referred or internally referred within the practice.

Participate in practice meetings, clinical governance, and quality improvement activities.

Manage referrals, investigations, results, and follow-up care in a timely manner.

Support safeguarding responsibilities for children and vulnerable adults.

Maintain professional development and comply with GMC appraisal and revalidation requirements.

Qualifications Essential

Fully qualified GP with GMC registration Evidence of annual appraisal and revalidation (when appropriate) On the medical performers list Enhanced DBS post is subject to DBS checks Full UK driving licence

Desirable

Evidence of continued professional development Other post graduate diplomas

Experience Essential

Knowledge of NHS Primary Care systems and structures Knowledge of QOF and Enhanced Services Experience working with the EMIS clinical system High standards of record keeping Strong understanding of NICE and local prescribing guidelines Effective time management Excellent communication skills, both verbal and written

Desirable

Understanding the needs of the local practice population Experience working in a General Practice/Primary Care Setting
