Job summary
This role forms part of the Fairer Practice Local Enhanced Service (LES) and is focused on reducing health inequalities, improving access to primary care, and supporting individuals with complex social, physical, and mental health needs.
You will work collaboratively with multidisciplinary teams, community organisations, social care services, and voluntary sector partners to improve health outcomes for people facing significant barriers to accessing healthcare.
