About the Role

Harrold Medical Practice has exciting opportunity for a clinical pharmacist into our practice team on a part-time basis. The role is offered on a part-time flexible basis, 1 2 sessions per week, between Monday and Friday. Remote working would be considered.

Main duties of the job

Our clinical pharmacist will work ensuring safe, evidence based, and cost-effective prescribing. You will also be responsible for offering support to general practice staff in relation to prescription and medication queries as well as conducting medicines reconciliation on transfer of care.

Responsibilities of the role will include:

Work as part of a multidisciplinary team in a patient-facing role. Take on responsibility for areas of chronic disease management, working proactively with other healthcare professionals. Support the delivery of the new Network Contract Directed Enhanced Service (DES). Conduct structured medication reviews to proactively manage patients with complex polypharmacy. Provide clinical medicines expertise to both patients and practice teams. Improve prescribing practices across the Practice. Improve service and quality through mechanisms such as clinical audits. Interface with community and hospital colleagues to help resolve medication-related issues.

Person Specification

Qualifications Essential

About the Candidate You will be a qualified pharmacist with strong experience within primary care and have the following skills and qualifications: Essential qualifications, skills, and experience: Vocational 4-year master's degree in pharmacy 1 years pre-registration training Registered with the General Pharmaceutical Council Evidence of continuing professional development Covid vaccination training Good critical appraisal skills Experience of patient consultation Experience of working with GPs Good understanding of primary care structure and general practice Good clinical knowledge Able to apply clinical knowledge, appropriately, in a variety of practice settings or patient groups. Desirable qualifications, skills, and experience: Postgraduate diploma/MSc in clinical or community pharmacy, or equivalent Non-medical prescriber registration Advanced assessment and history taking skills or be willing to enrol in a current training pathway to develop these. Post-registration experience in primary, secondary or community pharmacy Experience of clinical pharmacy Knowledge of GP clinical computer system SystmOne
