The System Integration Lead provides strategic, systemlevel leadership to embed One You Kent lifestyle services Smoking Cessation, Weight Management and Lifestyle into local care pathways across Kent.
The postholder will work across Integrated Care Boards (ICBs), Primary Care Networks (PCNs), Local Authorities, VCSE partners and NHS providers to design, implement and sustain integrated referral pathways, ensuring prevention and lifestyle interventions are embedded as a routine part of care.
This role is outwardfacing and influenceled, focused on system engagement, pathway redesign, referral integration, and service growth in terms of scalability and income generation, enabling One You Kent to reach priority populations and maximise impact at scale.
Main duties of the job
The role builds and maintains strong relationships with ICBs, PCNs, local authorities, NHS providers, and VCSE partners, acting as a credible system leader for One You Kent across countywide and place-based forums. It influences system decision-making to embed lifestyle services within clinical and prevention pathways, ensuring alignment with population health priorities.
It leads on pathway design and service integration by identifying opportunities to embed smoking cessation, weight management, and wider lifestyle interventions into existing care pathways. The role co-designs and tests new referral pathways with system partners, using pilot and test and learn approaches to inform wider implementation and scale-up.
A key aspect of the role is supporting referral and digital integration, working with clinical and digital teams to embed referral mechanisms within systems such as EMIS and SystmOne. It ensures pathways are simple, accessible, and effective, while also supporting engagement and training to build referrer confidence and uptake.
The post holder contributes to business development and growth by identifying opportunities for service expansion, innovation, and investment, as well as supporting the development of business cases, proposals, and bids aligned to population health needs. They also work to strengthen partnership models that improve reach, sustainability, and social value.

They act as an outward-facing, influence-led system leader, representing One You Kent at countywide and place-based forums to support pathway redesign, system engagement, and service growth, including scalability and income generation.
The role leads on referral and digital integration, working with clinical and digital teams to embed referral mechanisms into systems such as EMIS and SystmOne, while ensuring pathways remain accessible, efficient, and responsive to need. It also supports engagement and training to build confidence and uptake among referrers.
In addition, the post holder contributes to business development by identifying opportunities for service expansion, innovation, and investment, and supporting the development of business cases, proposals, and bids aligned to system priorities. They define KPIs for new pathways, use data and insight to evaluate impact, and drive continuous improvement through clear reporting to internal and external stakeholders.
The role also focuses on capacity building and workforce support, enabling partner organisations to effectively use referral pathways, sharing best practice across the system, and providing line management to Stakeholder and Engagement Officers to ensure effective delivery and coordination.

Qualifications Essential
Degree or equivalent experience in public health, health management, social care, or related field
Desirable
Postgraduate qualification in public health, systems leadership or service design

Proven experience working at system or place level within health and care

Experience across ICS or place based partnership structures
