Job summary
The post has the following principal areas of responsibility;
*To run an oxygen assessment service for patients requiring long term oxygen therapy (LTOT)
*To provide hospital / community-based care and support for patients with respiratory disorders requiring LTOT
*To work collaboratively with all health care professionals to provide a seamless package of care for patients with chronic respiratory disorders.
*To advise management, facilitate early discharge and provide educational support for patients admitted with an exacerbation of their airway's disease.
*To work closely with the other members of the oxygen service team, Consultant Clinical Scientist, Senior Chief Respiratory Physiologist and Respiratory Support Team
*An essential requirement of this role is that you have access to your own vehicle
Main duties of the job
*To advise medical staff on the management of patients being discharged from hospital on oxygen.
*To work with other members of the Respiratory team to provide a holistic care package for patients with Respiratory conditions.
*To manage acutely ill LTOT patients in the community reducing the need for readmission to hospital where possible
*To review and establish requirement for oxygen in palliative patients and liaise with palliative care team
*Advising GPs / DNs, key care nurses about changes in management of patients on oxygen therapy
*To provide advice for primary and secondary care with regard to respiratory conditions
*Creates a supporting, caring environment whilst caring for patients in their own home
*To review information for patients and other health care professionals maintaining close links with all areas of primary care
*To develop policies/standards of practice against which quality of care can be measured.
*To lead and promote smoking cessation counselling and be actively involved in the supporting patients during this process in liaison with occupational health and other agencies.
*To participate in and support patients undergoing pulmonary rehabilitation
*To actively ensure the continuing implementation of changes to clinical practice to improve patient care working within professional guidelines.
*Participation in the development of guidelines for good practice.

Qualifications Essential
*Registered Nurse with valid NMC registration *Educated to degree level or equivalent experience. *Portfolio of certified / accredited post registration professional development.
Desirable
*Health Assessment post graduate qualification / willingness to work towards
Experience Essential
*Post registration experience with evidence of knowledge and development for the associated area of specialist practice. *In depth knowledge of specialist procedures required for role, with willingness to undertake further training/ education to achieve agreed level of competence. *Knowledge of standards and policies associated with role. *Experience of working in a multi-disciplinary and cross agency work environment. *Demonstrable understanding of the national / NHSE diagnostic/ therapeutic expectations / strategy around endoscopy services and can relate this to the role. * Demonstrable understanding of clinical governance / patient safety process's -- can relate this to the role in relation to their personal/professional accountability.
