Job summary <p><strong>Make a Real Difference During the Moments That Matter Most</strong><br>Join our vibrant Out-of-Hours Community Health Navigation team at St Martins Hospital, Bath, where no two shifts are the same and every call is an opportunity to <span class="single_desc_int_gb_word">support</span> someone in need.</p><p>Were looking for motivated, peoplefocused Community Health Navigators who thrive in a fastpaced environment.
