About this role

Job summary

The post holder will maintain and co-ordinate timely and appropriate interventions through mental health nursing, assessment for a caseload of service users of any age living in the community within the framework of CPA, and in line with the acute care pathway as outlined within the CMHT operational procedure.

This includes respecting individuality, values, culture and religious diversity of clients and colleagues and thus contributing to the provision of a service sensitive to these needs.

Main duties of the job

To demonstrate high-level communication skills.

Be a full member of the multi-disciplinary team (MDT).

Effectively manage own caseload accepting referrals from agreed agencies in accordance with agreed operational procedures.

Ensure compliance with all Care Programme Approach requirements as per Trust Policy.

To participate in the continuous improvement of the service provided by the CMHT.
