Job summary
A new and exciting role has become available at Heaton Medical Centre/Dalefield Surgery. We are looking to recruit a salaried GP for 6-8 sessions to work over both sites. Salary dependant on experience.
Heaton Medical Centre & Dalefield Surgery are part of the Chorley Roads network and have a combined practice size of approximately 19,000 patients. There are 7 partners 4 salaried GPs, 2 ACPs and 3 Paramedics. We have an experienced nursing team and excellent administration team. We are forward thinking and have a number of ARRS staff including Mental Health Practitioner, MSK, Paramedic, Pharmacists, Social Prescribers, Dietician and Care Co-Ordinators.
We are high achievers of QOF and our local quality contract.
Main duties of the job
Provide high quality personal medical services to the practice populationTo shape and organise services to reflect the needs of the community and practice populationTo promote the practice as a learning organisation including staying abreast of policy, guidelines and evidence and communicating and integrating this into practiceTo work effectively within the primary healthcare team, promote team working and take on a leadership role where appropriateTo take part in and promote quality improvement processes including clinical governance and audit

Job description Job responsibilities
The role will be 6-8 sessions to be agreed and will be over both sites to undertake a variety of duties including surgery consultations, telephone consultations, home visits, processing of prescriptions and dealing with queries/paperwork (docman) in a timely fashion.
The successful candidate will manage a caseload and deal with a wide range of health needs in a primary care setting, ensuring the highest standards of care for all patients.
Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation.
Assessing the health care needs of patients with undifferentiated and undiagnosed problems.
Recording clear and consultation notes.
Screening patients for disease risk factors and early signs of illness.
Developing Care Plans & Health Education
Other responsibilities include:
Awareness of relevant practice policies and procedures
Managing and ensuring all training is up to date for Safeguarding, Data Protection, Health & Safety
Attending surgery meetings & training

Qualifications Essential
Full GMC Registration Appraisals and revalidation history Full driving licence Excellent communication skills
Desirable
Familiar with EMIS clinical software and Docman
