Job summary <p><strong>Make a meaningful difference to the lives of people living with heart failure.</strong></p><p>We are seeking a committed and skilled Cardiac Rehabilitation Practitioner to enhance the quality of life of people living with heart failure. This role plays a key part in delivering high-quality, evidence-based cardiac rehabilitation services across both community and clinical settings.</p><p>You will lead and support Phase 3 heart failure rehabilitation classes, work directly with patients in their own homes, and contribute to achieving optimal symptom management and maximum medical therapy. The postholder will provide specialist assessment, advice, rehabilitation, and ongoing support, ensuring care is delivered in line with NICE guidance, BACPR standards, and local policies.</p><p>The role also involves supporting and instructing the exercise component of the cardiac rehabilitation programme, leading and contributing to patient support groups, and actively participating in the development and future growth of the service.</p><p>This is an excellent opportunity for a motivated practitioner who is passionate about improving patient outcomes and shaping high-quality heart failure rehabilitation services.</p><h5>Clinical Responsibilities</h5><ul><li>Deliver timely, highquality cardiac rehabilitation in line with clinical protocols, service priorities, and professional stand
