Job summary

Symphony Healthcare Services is seeking an experienced Long Term Conditions / Chronic Disease Nurse, ideally with a specialism / interest in Diabetes, to become part of their dedicated teams that work at Wincanton Health Centre.

This is an opportunity to work in a collaborative, multidisciplinary environment providing high quality, patient-centred care as part of a team of health professionals and administrative staff. The role will combine assessing undifferentiated patients in both face-to-face consultations and via telephone calls, using digital platforms to triage patient requests and resolve queries from the administrative staff.

This vacancy is a permanent part time contract, equalling up to 24 hours per week.

The pay rate for this role is £25 per hour.

This vacancy will close at midnight on 22nd June 2026.

Main duties of the job

You will be an experienced, NMC registered Nurse who, whilst acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of patient care.

You will be able to demonstrate the ability to make differential diagnosis using decision making and problem solving skills whilst making professional autonomous decisions for which they are accountable. You will also possess the ability to work independently and collaboratively, as part of a multi-disciplinary team, with other health care professionals and disciplines, supporting the delivery of policy and procedures.

An interest / specialism in Diabetes and other Long Term Conditions would be desirable.

Job description Job responsibilities

Manage a designated case load of patients with chronic conditions, visiting them in their own home or consulting with them at the practice. Interest / specialism in Diabetes advantageous

Work within the NMC Code To mentor junior staff ensuring that they are supported and continually developed. To develop the nursing services offered both for the practice and through SHS. To support clinical Supervision within the practice. Produce accurate, contemporaneous and complete records of patient consultation, consistent with legislation, policies and procedures Prioritise, organise and manage own workload in a manner that maintains and promotes quality Deliver care according to NSF, NICE guidelines and evidence-based care Assess effectiveness of care delivery through self and peer review, benchmarking and formal appraisal.
