About this role

Job summary

Do you have the knowledge and experience to support people living with dementia, including young onset dementia, and their families? Are you able to work autonomously as a named key worker, undertaking triage, assessment, and ongoing monitoring, while providing pre and post diagnostic support?

Can you deliver tailored, non-pharmacological interventions using a whole family approach, and confidently identify and manage both cognitive decline and non cognitive symptoms?

Are you experienced in working within a multidisciplinary team and collaborating with Primary Care, statutory, and voluntary/third sector services, with nurse prescribing skills (or a willingness to develop these) being desirable?

Main duties of the job

To operate as a Community Mental Health Nurse within the Older Adult's pathway within North Tyneside Memory Clinic, particularly Early Onset Dementia

To provide specialist skills, knowledge and experience for those identified as having MentalHealth problems.

To work as part of a Community Team which may be integrated, working collaboratively and in partnership with other services.

To be compassionate in meeting the needs of Service User's and their carers.

To be professionally accountable and responsible for service user care. To undertake arange of clinical / therapeutic interventions within the service user's home environment orother community settings.

To role model compassionate and inclusive leadership in order to shape the creation of a collective leadership culture within the trust. This means demonstrating a consistentleadership style which (a) engages, enables and empowers others (b) uses coaching to promote ownership of learning and quality improvement and (c) facilitates team working and collaboration within teams / departments and across organisational boundaries.

Job description Job responsibilities

To undertake specialist Mental Health Nurse role within the broader Mental Health Services for Older People department. To operate within a multidisciplinary framework and contribute meaningfully to the development of the service, and the provision of safe and effective care. Reportable to Team Manager.

Person Specification

Qualifications / Professional Registration Essential

Registered Mental Health Nurse Part 1 with Current NMC registration. Successful completion of a post qualification Preceptorship programme. Evidence of qualification to undertake the supervision, teaching and assessing of students (essential requirement for post, expectation this will be completed within an 18 month period unless post is bank only). Evidence of Continuing Professional Development.

Desirable

Leadership or management qualification Post-basic qualification at Diploma / Degree level Non-medical prescribing qualification or interest in working towards.

Experience and knowledge Essential

Relevant post qualification experience in Older Adult Mental Health Services. Experience of working in a Mentor capacity with relevant qualification or willing to work towards. Knowledge and application of psychologically based therapeutic interventions.

Desirable

Experience of working with people with mental ill health in a community environment. Caseload management and experience of working autonomously. Knowledge of or experience in coaching and mentoring practices and tools Knowledge of or experience in Quality improvement tools, techniques and methods

Other requirements Essential

It is an essential requirement of the role that the post holder has a valid driving licence and is either a car owner and able to use the car for work purposes or has a Trust lease vehicle which may be used for the role. However, the Trust would consider making reasonable adjustments to the role if necessary, to enable a disabled person to undertake the role.
