Job summary
Colton Mill and The Grange Medical Centres are seeking an enthusiastic and motivated General Practitioner to join our friendly, supportive and forward-thinking team in East Leeds. Applicants must hold MRCGP qualification, be GMC registered, included on the NHS Performers List, and have experience managing acute and chronic conditions within Primary Care. Confidence in delivering face-to-face, telephone and digital consultations is essential.
Sessions are available on Monday, Thursday and Friday, with practice opening hours between 8:00am and 6:30pm. The proposed start date is June 2026, with flexibility for the right candidate.
The role includes 12-minute appointments, approximately 25 patient contacts per day, shared home visit and on-call responsibilities, and use of SystmOne with a Total Triage model supported by SystmConnect. Main duties of the job
The main duties of the role include providing high-quality clinical care through face-to-face, telephone, online and home visit consultations; diagnosing and managing acute and chronic conditions; prescribing and reviewing medication safely; making referrals and arranging investigations; maintaining accurate clinical records using SystmOne; contributing to QOF, enhanced services and PCN initiatives; participating in safeguarding, audits and quality improvement activities; and supporting the supervision and development of GP trainees and other learners within the practice.

Colton Mill and The Grange Medical Centres are two purpose-built GP surgeries based in East Leeds, providing high-quality primary care services to a patient population of approximately 14,000. We are an Advanced Training Practice and proud members of the Cross Gates Primary Care Network (PCN), supporting a wider population of around 33,000 patients.
Our experienced, friendly and forward-thinking multidisciplinary team is committed to delivering safe, compassionate and patient-centred care. We provide a wide range of services including routine and urgent care, minor surgery, long-acting reversible contraception (LARC), care home support, and PCN extended access services.
We operate a Total Triage model using SystmOne and SystmConnect, enabling patients to access care through face-to-face, telephone and online consultations. Job description Job responsibilities
The post holder will be a qualified General Practitioner registered with the GMC and included on the NHS Performers Register. The successful candidate will provide safe, effective and compassionate patient care within a supportive multidisciplinary team environment across Colton Mill and The Grange Medical Centres.
The role involves delivering high-quality clinical care through face-to-face, telephone, online and home visit consultations, managing both acute and chronic conditions, and working autonomously within professional boundaries while recognising when to seek support and guidance.
Main Duties and Responsibilities
Provide high-quality clinical care in line with GMC standards, national guidance and practice protocols.
Deliver consultations via face-to-face, telephone, online systems (SystmConnect) and home visits as required.
Diagnose and manage acute and chronic medical conditions using evidence-based practice.
Request, interpret and act upon investigations, pathology results and diagnostic tests.
Make referrals to secondary care and other healthcare professionals, including urgent and fast-track referrals where appropriate.
Prescribe safe, effective and appropriate medication in line with current prescribing guidance and legislation.
Maintain accurate, contemporaneous clinical records using SystmOne.
Participate in routine and urgent care delivery, including same-day/on-call and home visit responsibilities within the duty team structure.
Contribute to QOF, enhanced services, PCN initiatives and quality improvement schemes.
Undertake clinical administration including workflow management, correspondence and repeat prescription management.
Work collaboratively with the wider multidisciplinary team including pharmacy, nursing, community and mental health services.
Participate in audits, clinical governance, safeguarding and significant event analysis activities.
Support and supervise GP registrars, medical students, nursing students and other learners within the practice.
Attend and contribute to practice, PCN and MDT meetings.
Promote health education, self-care and preventative healthcare initiatives for patients.

Qualifications Essential
Fully qualified General Practitioner Registered with the General Medical Council (GMC) Included on the NHS Performers Register Licence to practise in the UK Evidence of ongoing Continuing Professional Development (CPD) Understanding of clinical governance, safeguarding and quality improvement Excellent communication and interpersonal skills Ability to work independently and as part of a multidisciplinary team Competent IT skills and ability to use electronic clinical systems
