About this role

Job summary

West Farm Surgery is a small, friendly practice based in North Tyneside. Due to a restructure of our clinical team we are looking for an advanced clinical practitioner to join us. You will be responsible for managing and providing care for the presenting patient with undifferentiated, undiagnosed conditions, from initial history taking, clinical assessment, diagnosis, treatment, prescribing and evaluation of care. To include acute, minor and long term conditions in line with existing practice and national policies and guidelines.

The post is for 25 hours per week and we can have some flexibility regarding the days/hours worked.

Main duties of the job

To see patients independently, assess, diagnose and treat patients that request same day clinical access, using skills in history taking, problem solving and clinical decision making to establish a diagnosis and management/treatment plan

If appropriate, refer patients to another healthcare professional, for further assessment/treatment or management. This will be done in a timely manner dependent on the severity of the presenting complaint, i.e. immediately, same day, rapid access etc

As an independent prescriber: to prescribe safely, effectively and appropriately for the condition in line with practice, local and national guidelines and current evidence based legislative frameworks

About us

Our team is very supportive of each other and have a great team ethos. We have regular meetings and also meet over lunch, where there is opportunity to talk through cases and seek advice from colleagues.

The practice team consists of 2 GP partners, 1 non-clinical partner, 3 Salaried GP, 1 Pharmacist, 1 pharmacy technician, 1 social prescriber, practice nursing team and a knowledgeable and supportive reception and admin team.

We are also a training and teaching practice for GP Registrars, 1st, 2nd, 3rd and 5th year medical students.

We are an open and honest practice and encourage all team members to bring ideas that can make our patients experience better and improve our teams job satisfaction.

We are an active member of the NW North Tyneside PCN. We have access to a variety of health care professionals through the network and our patient's access these at our local hub a short walk from the surgery.

Locally we have a home visiting service which means our home visiting is low. We also work very closely with our frailty team at our aligned nursing home who support the practice in the long-term care of the residents.

Person Specification

Qualifications Essential

Advanced Care Practitioner MSc/Post Grad Diploma MSc/Post Grad diploma to include modules in advanced assessment and diagnostic reasoning, complex clinical reasoning Independent prescribing qualification

Experience Essential

Experience of working in healthcare in an autonomous practitioner role Evidence of competence in the clinical assessment, diagnosis and management of all age groups presenting with undifferentiated diagnosis Telephone consultations Face to face consultations

Desirable

Experience of working in the community, primary or urgent care Experience of mentorship, leadership, audit, and research in clinical practice. Familiar with using Systm1 and AccuRx/e-consultations
