Job Title:Specialist Stop Smoking Community Health & Wellbeing Coach
Contract Type: 2 year fixed term
Remuneration: £ 30,580
Hours of work:Fulltime (37.5 hours per week Monday to Friday)-May include up to 2 evenings and some weekend work

Accountable to: Health & Wellbeing Manager
Location:Hybrid but majority of work on site in Haringey community locations

Main duties of the job
As a dedicated and passionate Health & Wellbeing Coach delivering on our One You Haringey smoking cessation services, you will play a vital role in helping individuals improve their overall health and well-being.
The role will include:
Delivering stop smoking clinics and offering person-centred support to residents who want to quit smoking.
Dedicated capacity as a named smoking cessation advisor to engage and deliver interventions to Haringey residents from priority populations who experience barriers to accessing mainstream stop smoking support. The advisor would create a work plan with those expected community partners as part of their role.
As the role will involve delivering smoking cessation interventions, if required we will provide training up to Level 2 smoking cessation training which equips community advisors with the skills to provide evidence-based behavioural support, if required.

Working days for this role are:
Generally, Monday-Friday but will include up to 2 evenings per week and occasional weekend work (with time back during the week in lieu).
The role is predominately community based with an option for some working from home.

Actively engage communities who experience health inequalities through outreach in community, clinical and non clinical settings
Attend community settings such as hostels, severe mental illness SMI services and substance misuse services, to deliver stop smoking interventions (the list is dependent on Haringey Borough demand for their residents)
Act as a named link worker between the Stop Smoking Service and wider support services, including NHS Talking Therapies, mental health and substance misuse services, Adult Social Care, homelessness, refugee, asylum seeker and migrant health services, health visiting and school nursing (019 services), and community pharmacies.
Deliver stop smoking sessions face to face, remotely by telephone or online via a video platform.
Promote the stop smoking service to residents and health and social care professionals to increase uptake and retention
Deliver awareness sessions and brief interventions in partner organisations
Build and maintain strong relationships with healthcare professionals and partner agencies
Attend relevant MDTs and pathway meetings where smoking is a key risk factor
Support clear referral pathways across primary care, mental health, substance misuse, social care and voluntary sector services
Provide warm handovers and coordinated follow-up to improve engagement and retention
Support smoking status reviews and medication considerations with GP practices for prescription only medications
Person Specification
Experience Essential
Experience of working effectively in a team. Experience of working with people with complex needs (e.g., severe mental illness, substance misuse, homelessness).
Desirable
Previous experience of delivering smoking cessation interventions.
Qualifications Essential
A minimum of 2 A-Levels or an equivalent Level 3 Diploma. GCSE English Language at a minimum of a Grade C/4 (or equivalent Level 2 qualification).
Desirable
Degree in Public Health, Psychology, or related subject. NCSCT Level 2
