Job summary
The post holder will support the Haringey Proactive Service Manager in the development and delivery of the MACCT at a neighbourhood level, through strategic, operational and financial management support. The post holder will also collaborate with the clinical leads, administrative staff, partners at neighbourhood level and the Proactive Care Service Manager.
Main duties of the job
This is a new role in a developing neighbourhood model and will require flexibility from the post holder to actively feed into the developing neighbourhood team and ways of working to deliver on the below:
To Improve outcomes for our most complex residents.
To ensure the development, provision, and maintenance of high quality efficient and effective services.
To collaborate with partners and multi-agency stakeholders to maintain relationships and ensure integrated working.
To support the Proactive Service Manager in providing matrix management in the Multi-agency Team through supervision, maintaining relationship with partners and organisational leads to support delivery of an integrated health service at a neighbourhood level and build strong working relationships to support good integration across partners.
Demonstrable experience managing and influencing senior clinical leaders across multiple organisations, with the ability to balance professional autonomy, differing governance arrangements and organisational priorities to deliver integrated neighbourhoodbased care
To support staff wellbeing through shaping Process and Policies.

Bringing together professionals from health, social care and the voluntary sector to support adults with frailty or complex needs, MACCT is widely recognised as the model on which the Integrated Neighbourhood Team (INT) can be built, demonstrating what truly joinedup, preventative and personcentred neighbourhood care can achieve.
These posts are funded for one year through neighbourhood testbed funding, providing an opportunity to build on the MACC model and support the development of a model Integrated Neighbourhood Team (INT)

Staff Management
Reporting to the Haringey Proactive Care Service Manager. The post holder will provide operational management and leadership for the Haringey Multi Agency Care and Coordination Team at their neighbourhood level (East or Central Haringey).
To recruit and develop competent, committed clinical, non-clinical and admin staff and to ensure that the appropriate levels of skill mix of staff are deployed to meet the needs of the service now and in the future.
To provide a framework within which individual members of staff are clear about their responsibilities and accountability and are involved in decision-making on the effective use of resources.
To ensure that all staff within the Haringey Multi-agency Care and Coordination Team are appraised by the substantive employing agency, complete their mandatory training and that education and development programmes are in place, empowering staff to fulfil their potential and maximise their contribution to the service.
To ensure compliance with all Trust Human Resource Policies.
To ensure that staff within Haringey Proactive Care have regular feedback about their performance and have agreed, and have active, personal development plans in place that meet their personal as well as ACS and Trust objectives and the ICB KPIs. All staff has an agreed job plan that is consistent with their grade and specialty.
To consistently review and develop staff within the Multi-Agency Care and Coordination Team and identify new ways of working to ensure services are patient-focused and outcomes improved.
To develop effective workforce planning strategies including the undertaking of multi-professional re-profiling and a review of recruitment and retention procedures.
To ensure compliance of the Trusts complaints procedure and to coordinate the response to all complaints relating to Haringey Proactive Care ensuring that lessons are learned and good practice shared across the Trust and partner organisations.
To manage sickness absence within agreed targets and to ensure accurate records are kept in line with Trust policy.
Managing/Modernising Services:
To ensure all systems, policies, and processes within Haringey Proactive Care are patient-centred and in accordance with organisational and national objectives.
To achieve Trust-wide and service goals and objectives, involving established partner organisations within the overall performance and reporting as required.
To be responsible for the effective and efficient use of all resources within Haringey Proactive Care with regular stock takes and reviews of products ordered.
To provide managerial leadership to ensure that all resources are managed within the available and agreed financial budgets, ensuring adequate control of human and/or financial resources.
To be responsible for driving costs down and improving productivity within Haringey Proactive Care.
To identify and facilitate the delivery of CIPs schemes to ensure that financial targets are delivered.
To participate in the development and implementation of the Trusts business plan, ensuring that Haringey Proactive Care Service Development Plans are supportive of the strategic long and short-term objectives of the Trust and those of partner agencies.
To work closely with the Adult Community Services and other Partner agencies management teams to ensure the corporate goals are achieved and that the service contributes to the goals of partner organisations.
To agree service level agreements with the local Commissioners and Partner Agencies in respect of the provision of high-quality services to ensure that objectives are fulfilled, and high professional standards are maintained.
To be responsible for the delivery of specific service objectives and standards of Haringey Proactive Care against which performance can be measured (KPIs)
To ensure that services are delivered to CQC standards and that clinical governance processes provide assurance to the ACS senior management team and Partner organisation.
To work with staff to implement the Trust and Partner organisations. Clinical Strategic Direction to continuously improve the quality and timeliness of services to enhance the patients experience, reduce waiting times and minimise inefficiencies.
To ensure that patients are involved in the development of services to improve outcomes.
To demonstrate the attainment of measurable quality standards (including those set by NICE) within Haringey Proactive Care benchmarking against other NHS Trusts.
To take on the core corporate lead as required for ICSU-wide projects or work streams.
To participate in the enactment of any external inspections of services and ensure the development of any subsequent action plan and its progress.
To produce high-quality bids for service developments.
To manage the implementation and further roll-out of national standards and strategies.
To manage all risk and health and safety issues within Haringey Proactive Care.
Communication:
To ensure that service meetings are held on a regular and frequent basis to discuss and plan the business and activities of the Service(s).
To ensure that regular meetings are held within Haringey Proactive Care and that team briefing meetings are held for all staff.
To ensure that all staff are regularly updated about matters relating to ACS, the Trust, partner organisations and ICS.
To contribute to communication and public relations within and external to ACS by ensuring good communication flows across all partner organisations.
To encourage a culture of staff involvement, openness and psychological safety.
To identify risks and potential incidents that affect business continuity and escalate to relevant members of staff as necessary.
To deal effectively with verbal and written complaints by taking rapid action to rectify difficulties immediately wherever this is possible. To investigate and formulate responses to written complaints
within Trust & partner organisations timescales and ensure that action is taken to avoid recurrence where appropriate and learning shared and acted on. To report trends in complaints.
To represent the MACCT at external meetings with ICB and other agencies when directed.
Contract & Activity Management:
To review the Patient Tracker Lists (PTL) and ensure all data is accurate in collaboration with administrative and clinical lead staff.
To provide monthly performance reports to the Haringey Proactive Care Service manager and contribute to board highlight reporting and quarterly performance reviews.
To ensure referrals are triaged within agreed timescales and support the triage by maintaining a regular slot on the triage rota.
To ensure that performance against waiting time targets is closely monitored, and appropriate action plans are developed and agreed.
To be responsible for the timely production of standard and ad hoc information requests to enable ACS service planning and delivery against targets and service agreement levels.
To maintain up-to-date service information on the GP website.
To be responsible for ensuring data quality in service areas.
To ensure corrective action is taken where data quality problems are detected.
To ensure that all activity within Haringey Proactive Care is recorded accurately to ensure compliance with Trust's financial priorities.
