Ongar Health Centre is looking for a motivated and experienced clinical pharmacist to join our friendly and supportive team.

Main duties of the job
Key responsibilities will include:
Deliver high-quality clinical pharmacy services to patients Undertake structured medication reviews and medicines reconciliation Act as an independent prescriber to manage & optimise patient treatment plans Support safe, cost-effective prescribing in line with local and national guidance Manage long-term conditions and contribute to chronic disease management Respond to medication queries from patients and clinical staff Work collaboratively with GPs, nurses, and the wider practice team Support quality improvement and prescribing audit initiatives

The successful candidate must be able to undertake a range duties to support the multidisciplinary team, be able to work as part of a team, have the ability to organise and prioritise their own workload on a day-to-day basis. Essential
An appreciation of the nature of GPs and general practice An appreciation of the nature of primary care prescribing Experience and awareness of common acute and chronic conditions that are likely to be seen in general practice Excellent communication skills with a clear and polite telephone manner Work in a polite, confidential, and discreet manner, respecting patients at all times Confident to work as part of a team and be able to think on your feet independently Strong IT and administration skills Previous experience of SystmOne clinical system is desirable but not essential as training will be given
Qualifications Essential
GPhC registered pharmacist Hold the GPhC independent prescribing qualification
