Job summary

We are seeking a highly motivated and experienced Tissue Viability Nurse to join our Community Nursing Service as a Band 7 specialist practitioner. This is an exciting opportunity to lead on improving patient outcomes through expert clinical practice, education, and service development across community settings.

The post holder will play a pivotal role in reducing the incidence of pressure ulcers, enhancing wound care standards, and supporting the development of the community nursing workforce.

Main duties of the job

Provide specialist advice and clinical expertise in tissue viability, including complex wound management and leg ulcer care Lead initiatives to reduce the incidence and severity of pressure ulcers across community services Support and develop the knowledge and skills of community nursing teams through education, mentorship, and clinical supervision Deliver practice development programmes to enhance evidence-based care Work collaboratively with GP practices, care homes, and multidisciplinary teams to improve patient outcomes Undertake audits, service evaluations, and contribute to quality improvement initiatives Act as a clinical resource for staff managing patients with complex tissue viability needs

Job description Job responsibilities

Purpose

To provide specialist tissue viability assessments to patients in CSH Surrey their homes, Community Hospitals and clinics, who have complex or non healing wounds, ensuring the delivery of a high quality, clinically robust, evidence based Tissue Viability Service.

This is both an office and community based role to provide advice and support to Nursing Homes and 36 GP Practices within Surrey Downs Clinical Commissioning Group.

To provide specialist advice to CSH to guide commissioning of cost-effective products and dressings, to develop innovative practices and to act as an expert resource for wound management/Tissue Viability within CSH Surrey.

To provide training programmes for practice development, creatively delivered for staff within CSH Surrey who will be making clinical decisions for patients with compromised tissue viability conditions, aimed at improving clinical outcomes and patient wellbeing.

To provide quality monitoring and audit processes related to tissue viability, working closely with the CSH Quality and Governance team.

To support the Head of Tissue Viability Service within CSH - including the management and development of junior Band 7 Tissue Viability Nurse.

Principal duties and Responsibilities

Clinical Practice

Act as a professional role model to all staff, both internal and external on behalf of CSH Surrey, working within the multi disciplinary team, taking the lead and developing the Tissue Viability service.

To provide specialist advice and support as requested on all aspects of tissue viability

Demonstrate clinical expertise in tissue viability being accountable for tissue viability assessments of patients referred to the service

Recommend a prescribed course of treatment/care using researched based evidence and principles of best practice for this patient group

To autonomously assess and manage own patient caseload of complex cases

Treat all patients as individuals and develop patient centred care plans, respecting their privacy and dignity at all times

Involving, supporting, informing and educating family/carers

To ensure effective communication and liaison between the Community Integrated Teams, Community Hospitals, the wider multi-disciplinary team and GPs

Research

To keep up to date with current research and developments within the speciality including NICE guidelines in order to make to confident in making recommendations to Commissioners on best practice

To contribute to the research and comparison regarding the effectiveness of various dressings, equipment care and treatments within the speciality

To evaluate new treatments using current research, monitoring and identifying risks

Participate in research and development opportunities as appropriate

Education and training

To initiate, plan and provide education and training across CSH

To support and develop junior Band 7 Tissue Viability Nurses

To develop skin champions for all Community Services to ensure that they have up to date knowledge and skills on best practice to be disseminated within their teams.

To work with other Tissue Viability specialists within Surrey to ensure best practice is maintained

Provide support and an appropriate learning environment for both pre and post registration students as required

Involving, supporting, informing and educating family/carers

To take every opportunity to promote health and wellbeing of the patient

Act as an assessor and mentor to qualified and unqualified staff

Quality

To ensure CSH has access to the agreed updated Surrey wide wound care formulary

To promote and monitor the use of appropriate tissue viability documentation, including audit and evaluation

Monitor the quality of the service through audit

Provide specialist knowledge and advice to influence the CSH Surrey strategic agenda

Maintain clear and comprehensive, signed and contemporaneous records according to procedures

Management

The Tissue Viability Service Co-owner is to work within all CSH Surrey localities

To give leadership support to the junior Band 7 TVN colleagues.

Senior band 7 Tissue Viability Nurse can provide line management responsibility for entry level band 7 Tissue Viability Nurses as agreed with Head of Tissue Viability to support professional development and leadership progression

Identify workforce planning issues and actively participate in the recruitment, selection and retention of clinical staff

Be responsible for ensuring that policies and procedures and standards of care, are adhered to at all times

Manage effectively and efficiently any budget that is within the remit of this post

Provide assistance with the resolution of complaints within the clinical specialty, or sphere of responsibility

To actively participate in and facilitate clinical supervision

Have the ability to negotiate and work effectively across all agencies for the maximum effectiveness of care

Be aware of and act upon when necessary, procedures that are in place to protect vulnerable individuals

Lead and implement the CSH Surrey Quality & Governance Strategy within your practice area, facilitating and instigating clinical audit and monitoring of care

Lead and implement the CSH Surrey Risk Management Strategy within your practice area, ensuring that all processes are adhered to Maintain own professional and clinical integrity in line with NMC guidelines

Undertake any other such duties as may be required from time to time as are consistent with the responsibilities of the post

Be responsible for individual timely data entry and responsible for the tissue viability teams data entry being up to date.

Work as part of the team undertaking lead responsibilities as negotiated and agreed with the person to whom you are responsible, be accountable in the provision of a high standard of individual quality patient care, within the agreed guidelines.

To work without direct supervision and to develop and maintain effective communication and liaison systems

Maintain own professional and clinical integrity in line with NMC guidelines

Be responsible and maintain timely patient care

To work in co-operation and partnership with all other professionals and agencies involved in the care of the patient

Person Specification

Experience Essential

To have held recent post registration experience as a Specialist Practitioner/ Advanced Practitioner in Tissue Viability in a community setting Understand pressure ulcer surveillance methodologies Experience in managing the needs of complex patients in a community setting Experience of developing strategies, clinical guidelines and policies The ability to challenge and discuss poor practice with other professionals Experienced mentor at pre and post registration level Experience of nurse leadership and management Sound knowledge of current issues and legislation within the NHS Previous experience of team management and multidisciplinary working Participation in research and clinical audit Clinical Governance & Risk Management agenda Experience of appraisal and clinical supervision

Qualifications Essential

1st level registration educated to Level 3 with current NMC registration. Teaching/mentor qualification Evidence of continued post graduate professional development, with courses relevant to area of work Relevant specialist courses Evidence of team Management

Desirable

Willingness to develop areas of specialist interest Management qualification/or willingness to work towards Specialist Practitioner Qualification/Advanced Practitioner at Degree/ Masters level in Tissue Viability Nurse prescribing qualification V100
