Job summary
We have an exciting opportunity for a Registered Nurse to join our Rehabilitation Ward at Woking Community Hospital. Alexandra Ward provides comprehensive, integrated rehabilitation for adults who are recovering from illness or injury, or who are experiencing an increase in frailty and deteriorating functional abilities.
The Band 5 nurse role is a key part of the multi-disciplinary team providing 24-hour care and therapeutic support to patients who are either being discharged from an acute hospital or being transferring directly from the community.
We are looking for an enthusiastic and motivated nurse, who cares about the experience of the older person in hospital, and wants to ensure that individuals are treated with care, dignity and respect.

Main duties of the job
The Registered Nurse role works across a seven-day week, working shift patterns of both long days and nights. This promotes a consistent approach to the clinical management of the patients and rehabilitation on the ward by all staff.
You will be supporting the Ward Matron, and Sister/ Charge nurse role in in the co-ordination and monitoring of person-centred care.
As a member of the multi-disciplinary team you will work without direct supervision, supporting the team in promoting health, independence and maximising the quality of life of patients.
You will be supervising Healthcare Assistant and students where necessary and co-ordination of ward management.
You will regularly act as Nurse in Charge for the ward in the absence of the Ward Matron, or Sister/ Charge Nurse acting as main point of contact for any staffing problems occurring on site within the shift and the appropriate escalation of these to the Senior Manager on call.

Job description Job responsibilities
To work with frail older adults who have varying medical needs, including confusional states, mild cognitive impairment, dementia, post-stroke, falls, post-operative recovery of fractures, and general reduced mobility.
To have responsibility for assessing, planning, implementing and evaluating programmes of care with minimal supervision.
To promote a high standard of evidenced based nursing care and dignity for patients at all times.
To assist senior nurses with ensuring patients nutritional status is maintained and maximised to facilitate patient recovery and well-being
To assist senior nurses with ensuring a clean ward environment for patient care and best practice in infection control.
To support the Ward Matron and Sister/ Charge Nurse to ensure performance indicators are achieved e.g. infection rates, average length of stay, delayed discharges.
To participate in clinical governance activities as required eg ward audits, and support the Ward Matron and Sister/ Charge Nurse in the implementation of action plans.

Knowledge and Skills Essential

Qualifications Essential
Registered Nurse (RN) or Registered Mental Nurse (RMN) and has current live PIN on NMC register

Mentorship and assessment qualification Degree or Diploma in relevant field (or equivalent experience)
Experience Essential
Previous experience of working within a ward environment Interest in working in rehabilitation Interest in working with frail older adults. Minimum six months NHS or equivalent health care system experience is essential

Experience of working with older adults, including patients with confusional states/dementia. Experience of working with post-stroke patients/ neuro-rehabilitation.
