This role requires you to be responsible for the clinical oversight, operational coordination, and ongoing development of the UCR service. As a Clinical Lead Nurse, you will support the delivery of high-quality, responsive care to patients in their own homes, helping to prevent unnecessary hospital admissions and facilitating timely interventions for those with urgent healthcare needs. This full-time position involves providing clinical leadership and operational oversight for the UCR team. Your role will include managing the day-to-day delivery of the service to ensure safe, effective, and patient-centred care. Among your main responsibilities, you will undertake advanced assessment, planning, implementation, and evaluation of care for patients with complex needs. You will act as an autonomous practitioner, make sound clinical decisions, and delegate appropriately to support, supervise, and develop team members while promoting high standards of practice.

This is a full-time position working 37.5 hours per week, ideally 08:00 to 16:00 Monday to Friday, with occasional weekend working as required.
Main Responsibilities Provide clinical leadership and operational oversight for the UCR team. Manage the day-to-day delivery of the service, ensuring safe, effective and patient-centred care. Undertake advanced assessment, planning, implementation and evaluation of care for patients with complex needs. Act as an autonomous practitioner, making sound clinical decisions and delegating appropriately. Support, supervise and develop team members, promoting high standards of practice. Ensure all documentation is completed in line with NMC standards and HCRG Care Group policies. Monitor service performance and contribute to continuous improvement initiatives. The Ideal CandidateThe ideal candidate will have:
Current registration with the Nursing and Midwifery Council (NMC) Evidence of continuing professional development and education at degree level or equivalent Leadership or management qualification, or willingness to work towards one Teaching and assessing qualification or equivalent experience Significant post-registration experience, including community nursing experience Experience of leading teams and managing services Experience of caring for patients with complex and long-term conditions Strong clinical decision-making and organisational skills A full UK driving licence and access to a vehicle for work purposes
Person Specification
Qualifications Essential
Current registration with the Nursing and Midwifery Council (NMC) is mandatory. Evidence of continuing professional development and education at degree level or equivalent is required, along with a leadership or management qualification, or a willingness to work towards one. A teaching and assessing qualification or equivalent experience is necessary. Significant post-registration experience, including community nursing experience, and experience of leading teams and managing services are required. The ideal candidate should possess strong clinical decision-making and organisational skills, and must hold a full UK driving licence with access to a vehicle for work purposes.
