Job summary

We are looking for Salaried GPs to join our established and supportive primary care team at Holderness Health. This is a flexible opportunity with sessions - up to 8 per week - negotiable, including a regular Enhanced Access session one day per week, offering additional flexibility for clinicians interested in extended hours working.

The role is ideal for an experienced GP or someone newly qualified who is looking to develop their career within a forward thinking practice.

The post holder will provide high quality, patient centred general medical services to our registered and temporary patients, working collaboratively with the wider clinical and nonclinical team to meet a broad range of health needs.

Main duties of the job

You will deliver high quality, patient centred GP services including face to face and telephone consultations, home visits, Duty Doctor cover and weekly participation in Enhanced Access services. You will manage acute, chronic, complex and undifferentiated conditions and develop care plans with patients.

You will prescribe safely in line with guidance, keep accurate clinical records, and contribute to QOF and quality improvement activity. Working collaboratively with the multidisciplinary team of GPs, Advanced Nurse Practitioners, First Contact Physiotherapists, Proactive Care Team including Social Prescribers and Mental Health Worker, you will be committed to delivering excellent patient care and positive patient outcomes, with a focus on supporting continuity of care for our most complex patients.

We are looking for a fully qualified GP with GMC registration, on the NHS Medical Performers List or nearing completion of training. You will have strong clinical judgement, excellent communication skills, good organisation and a compassionate, professional approach. You should be able to manage workload effectively, work independently and value teamwork, prevention and reducing health inequalities. Commitment to patient care.

We operate from seven sites, the successful candidate must be willing and able to travel between locations as required. Job description Job responsibilities

The post-holder will manage a caseload and deal with a wide range of health needs in a primary care setting, ensuring the highest standards of care for all registered and temporary patients.

Clinical Responsibilities:

In accordance with the practice timetable, as agreed, the post holder will be available to undertake a variety of duties including surgery consultations, telephone consultations and queries, visiting patients at home, checking and signing repeat prescriptions and dealing with queries, paperwork and correspondence in a timely fashion.

Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation

Assessing the health care needs of patients with undifferentiated and undiagnosed problems

Screening patients for disease risk factors and early signs of illness

In consultation with patients and in line with current practice disease management protocols, developing care plans for health

Providing counselling and health education

Admitting or discharging patients to and from the caseload and referring to other care providers as appropriate

Recording clear and contemporaneous consultation notes to agreed standards

Collecting data QOF data and other data required by the practice

Contributing to evaluation/audit and clinical standard setting within the organisation

Participating in Significant Event Analysis

Compiling and issuing computer-generated acute and repeat prescriptions (avoiding hand-written prescriptions whenever possible)

Prescribing in accordance with the practice prescribing formulary (or generically) whenever this is clinically appropriate

Participate in training plans of medical students as required

In general the post-holder will be expected to undertake all the normal duties and responsibilities associated with a GP working within general practice

Other Practice Responsibilities:

Awareness of and compliance with all relevant practice/CQC policies/guidelines, e.g. prescribing, confidentiality, data protection, health and safety

Undertaking Duty Doctor responsibilities on a rotational basis

Contributing towards extended hours provision on a weekly basis

Supporting colleagues with the smooth running of the surgery by providing cover and assistance, in particular during periods of illness, annual leave or absence for other reasons

Personal/Professional Development:

Attend courses and study days to ensure continuing professional development, taking responsibility for own development

Participate in individual performance review including annual appraisals and internal peer review

Comply with all Health and Safety and infection control requirements
