Job summary

An exciting opportunity has arisen to join us at South Tyneside and Sunderland NHS Foundation Trust as a Peg/Enteral Nurse Specialist to meet the increasing requirement across South Tyneside and Sunderland.

This post will focus particularly on the delivery of national and trust standards in the provision of Enteral Nutrition in Adults. The post is for 37.5 hours per week. The successful candidate requires excellent organisational and communication skills.

Although working as part of a team, you will be working autonomously to manage your own caseload of patients, carrying out nurse-led clinics and inpatient reviews.

You will also provide a point of access for specialist advice and support for patients, carers and the multidisciplinary team within both primary and secondary care. You will be someone who has a passion for making a difference and putting the patient first and will strive to deliver the best service for patients. You will be supported by an excellent wider team of specialist nurses, AHPs and medical staff from both hospital sites.

Essential Criteria:

Current NMC registration.Degree Level Education.High level of experience working with PEG/Enteral nutrition or working within Gastroenterology currently.Clinical Skills Qualification or willingness to complete.Teaching qualification (Mentoring).

Main duties of the job

Working closely with the MDT, you will care for elderly patients with a range of acute medical needs, often with complex health and social care requirements including:

The successful candidate will be able to demonstrate excellent communication skills, team working skills and will be dedicated to improving the patients' experience. You will need to demonstrate continuing professional development and a strong commitment to develop others and work closely within a multi-disciplinary team.

The post holder will run nurse led clinics where the post holder will co-ordinate treatment plans in conjunction with medical staff, this may include but is not limited to elective tube replacements, troubleshooting, NG tube insertion and verification and Nutrition training and support.

The post holder will be expected to support the Matron, their colleagues and the specialty team to maintain a patient focused positive experience and environment.

As a specialist nurse within the gastroenterology service, the post-holder will:

Demonstrate leadership in contributing to a culture of patient safety and continuous improvement in line with the clinical governance framework. Be accountable for provision of a specialist nursing service, using clinical skills in the assessment, diagnosis, prescribing and delivery of treatment plans to support all aspects of patient care across the pathway. Co-ordinate the provision of services to patients within specialist area, ensuring continuity of care across the Trust and between primary and secondary care. Play a key role in facilitating hospital discharge and preventing avoidable hospital admissions As an expert practitioner, be accessible to provide education and training and clinical advice and support to Trust staff, primary care staff, patients and relatives/carers. Participate in / lead on audit and research relevant to the needs of specialist area Participate in service development within gastroenterology across the Trust. Contribute to the achievement of national and local standards/targets. Support the Matron or team lead to facilitate coordination and delivery of a service which is both efficient and evidence-based.

Person Specification

Qualifications Essential

Live 1st level NMC registration. Relevant degree. Clinical skills or willingness to complete.

Desirable

Qualification in specialty. Teaching Qualification.

Skills and Knowledge Essential

Evidence of continuous professional development must demonstrate development of knowledge and skills in Gastroenterology/PEG/ nutrition and its management as part of CPD. Basic IT skills. Proven leadership skills. Proven communication skills including experience of handling complex relationships and interpersonal dynamics. Knowledge of how to facilitate learning. Knowledge of current professional and Clinical Governance issues. Knowledge of research and clinical audit. Venepuncture IV drug administration and cannulation Mentorship skills. Evidence of competency in relation to nursing practices appropriate to the post. Time management. Ability to assess, deliver, evaluate and benchmark quality care.

Experience Essential

Nursing experience at band 6 or above. Recent experience of care delivery. Knowledge and experience of Gastroenterology/PEG nutrition care. Experience of leadership within a team. Evidence of innovation in practice. Teaching experience.
