In this role, youll deliver high-quality, evidence-based respiratory nursing care to patients with chronic respiratory conditions, helping to improve quality of life, promote independence, prevent avoidable hospital admissions, and support timely discharge from acute services.</p><p style="margin:0px;font-size:inherit;padding:5px 0px;">Youll work closely with senior clinicians, the wider multidisciplinary team, and primary care colleagues, contributing to proactive case management and health promotion initiatives aligned with neighbourhood and community care models.</p><p style="margin:0px;font-size:inherit;padding:5px 0px;">Your base will be Livingstone Hospital, Dartford, with regular travel across Dartford, Gravesend and Swanley. As this is a community-based role, a UK driving licence and access to a car for work purposes is essential.</p>
Main duties of the job <p style="margin:0px;font-size:inherit;padding:5px 0px;">As a Band 6 Community Respiratory Nurse, you will:</p><ul style="margin-top:0px;margin-bottom:10px;"><li style="font-size:inherit;"><p style="margin:0px;font-size:inherit;padding:5px 0px;display:unset;">Manage a defined caseload of patients with chronic respiratory conditions such as COPD and asthma, under the guidance of senior clinicians</p></li><li style="font-size:inherit;"><p style="margin:0px;font-size:inherit;padding:5px 0px;display:unset;">Undertake holistic patient assessments, deliver agreed clinical interventions, and support patients with self-management plans</p></li><li style="font-size:inherit;"><p style="margin:0px;font-size:inherit;padding:5px 0px;display:unset;">Monitor patient progress and escalate concerns appropriately to support admission avoidance and early discharge</p></li><li style="font-size:inherit;"><p style="margin:0px;font-size:inherit;padding:5px 0px;display:unset;">Provide respiratory education and health promotion to patients, carers, and colleagues</p></li><li style="font-size:inherit;"><p style="margin:0px;font-size:inherit;padding:5px 0px;display:unset;">Work collaboratively with multidisciplinary teams across community, primary, and acute ca
