Main duties of the job As a Band 5 Occupational Therapist, you will carry out therapeutic assessments, diagnosis and treatment using person-centred goals within a community-based, multi-disciplinary team. You will be responsible for your caseload and maintaining up-to-date records using SystmOne. The role involves collaborating with Community Support Workers to deliver high-standard therapeutic programmes. You will be working in diverse environments including patient homes and residential or nursing homes. Responsibilities include providing evidence-based care through comprehensive assessments, care planning, rehabilitation, and discharge plans. The ideal candidate must hold a diploma/degree in Occupational Therapy, be a member of The Royal College of Occupational Therapists, have HCPC registration, a CPD portfolio, and effective experience in various practice areas. A valid driving license with business insurance is also essential.

Main Responsibilities:
To carry out therapeutic assessment, diagnosis and treatment with patients using person centred goals, who may have diverse presentations and a range of physical and psychological conditions within the context of a community-based, multi-disciplinary team. Responsibility for up-to-date record keeping within professional guidelines and HCRG Care group Record keeping policy - currently using SystmOne as our electronic clinical records system. To hold responsibility for own caseload and capacity working with Community Support Workers to deliver therapeutic programmes as part of the multi-disciplinary team. Provide an evidence-basedOccupationalservice of a high standard, to patients in a community setting by undertaking comprehensive assessments, reassessments, care planning, rehabilitation and therapeutic care and support.This will involve using clinical reasoning skills and knowledge of evidence-based practice to inform the appropriate intervention and developing comprehensive discharge plans from the service.Please see the job description attached for a full list of responsibilities.
The ideal candidate will have -
Essential:
Diploma/ Degree in Occupational Therapy
Membership of The Royal College of Occupational Therapists and/ or evidence of other professional liability insurance to practice
Registered with HCPC
Evidence of continuing professional development (CPD) recorded in a portfolio, as recommended by HCPC.
To have gained effective experience in a range of practice areas from student placements and/or previous recent experience as a Band 5 Occupational Therapist
To demonstrate knowledge of principles and practices within Occupational Therapy to include knowledge of medical conditions prevalent in the elderly such as Parkinsons Disease, Stroke, MS, COPD, falls, orthopaedic conditions, general ageing and diabetes.
Good working knowledge of typical work-related systems, procedures, equipment and technology
Knowledge and understanding of the legislation that applies to care and health work
Car driver with access to vehicle, business Insurance and be able to travel Independently around the Bath and North- East Somerset area
Desirable:
