Job summary
NHFT is contracted to provide integrated health care services to HMP Littlehey. Currently we are looking to recruit an innovative Band 7 long term condition lead to our existing team. The post holder will deliver the highest standard of care required to meet the diverse and complex healthcare needs of an adult male population. They will be passionate about all aspects of Nursing, have exceptional clinical skills and hold an NMP qualification. Demonstrating consistent exemplary standards of care and professionalism.
HMP Littlehey is a large Category C prison located in the picturesque village of Perry near to Grafham Water near Huntingdon in Cambridgeshire with free parking, staff restaurant and an onsite gymnasium.
Healthcare services are delivered from two modern well equipped healthcare centers by a multi-disciplinary team who have, in the past, won the prestigious WHO European Best Practice in Prisons Award. Main duties of the job
Clinical Care
Assess, monitor, and manage patients with long-term conditions such as diabetes, asthma, COPD, cardiovascular disease, epilepsy, and hypertension
Deliver structured LTC clinics and annual health reviews
Develop, implement, and review individualised care plans in line with national guidelines
Medication Management
Prescribe and administer medication in line with professional competence and prison policy. Ensuring safe medicines management
Support continuity of medication on reception, transfer, and release.
Support patients to self-manage their long-term conditions
Promote healthy lifestyle choices, including smoking cessation, diet, physical activity and patient education
Multidisciplinary Working
Work collaboratively with GPs, mental health services, substance misuse teams, allied health professionals, and prison staff
Oversee and supervise the wider healthcare team to support them in the management of LTC
Contribute to case reviews, MDT meetings, and care planning discussions.
Act as a patient advocate while maintaining security and safety requirements.
Documentation and Governance
Maintain accurate, timely, and confidential clinical records
Work in accordance with NMC Code, clinical governance frameworks, and prison healthcare policies
Participate in audit, service development, and quality improvement initiatives

Job description Job responsibilities
The Primary care and Long-Term Conditions (LTC) clinical lead working in a prison is responsible for delivering high-quality, evidence-based care to people in custody with chronic health conditions, ensuring care is equivalent to that provided in the community.
The role involves overseeing nursing staff while assessing, monitoring, and managing long-term conditions through regular clinics and reviews. You will also bedeveloping individualized care plans and supporting safe and effective medication management. You will work autonomously and as part of a multidisciplinary team, promoting self-management and health education while balancing clinical priorities with the constraints of a secure environment. The post holder will also play a key role in safeguarding, risk management, accurate clinical documentation, and ensuring continuity of care during transfers and on release.
