Job summary
An exciting opportunity has arisen for a band 6 nurse to join the SNOP team (Specialist nurses for older persons). The SNOP's are specialist nurses that are based throughout the trust including Acute care, DME, ED and the frailty unit .
The successful candidate will be required to holistically assess our older patients at the front door. As soon as our frail patients present to the accident and emergency department our aim is to reduce the risk of patients deconditioning whilst waiting for diagnostic tests and medical attention and a decision as to whether they are able to go home with support in the community or decision to admit. We strive to deliver a high standard of care to our patients keeping them at the heart of what we do.
We aim to facilitate discharge for those we can support with community services using a pragmatic approach such as rapid response, community matrons, NPOP's, age uk etc. We are looking for enthusiastic and motivated individuals who have an interest in frailty.
We will provide specialist training as well as the support from the MDT team. Successful candidates must be a minimum of 2 years qualified or have frailty experience.
Main duties of the job
Assess, plan, implement and evaluate care for older frail patients, offering advice and support to ward staff, patients and their families. Provide expert advice and clinical practice and be able to provide skills required for patient assessment e.g. catheter insertion, cannulation, ecg's etc. Participate in the education and development of other staff members. Work collaboratively and co-operatively with clinical colleagues to develop services and quality of care delivered. To ensure the management plan following CGA is commenced for all relevant patients following assessment and try and hit target to complete within 1hour of admission.
