We are currently seeking to recruit a Care Coordinator (37.5 HPW) to provide a comprehensive Admin/QOF/reception service to achieve an efficient and effective service in providing the best possible care to patients

Sunderland GP Alliance runs three medical practices at New Silksworth Medical Practice, South Hylton Surgery and Monument Surgeries.

Job description Job responsibilities

Maintain, update and validate patient records to ensure accuracy for QOF reporting. Ensure correct coding for disease registers (e.g., asthma, COPD, diabetes, LD, SMI). Read incoming clinical correspondence (e.g., hospital letters) and code appropriately. Track QOF indicators, monitor performance and identify gaps. Identify eligible patients for QOF reviews and send recalls via letters/SMS. Follow up on nonattenders and maintain recall logs. Maintain disease registers. Perform clinical searches to identify patients missing QOF indicators.

The Care Coordinator (QOF Focus) supports the delivery of safe, effective and highquality care by coordinating patient reviews, managing recall systems and ensuring accurate data capture aligned to the Quality and Outcomes Framework (QOF). The role involves working closely with clinical teams, maintaining disease registers, ensuring robust recall processes, and contributing to QOF performance and compliance

Key Responsibilities:

Data Management & Coding QOF Performance Coordination Patient Recall & Review Management Disease Register & Care Coordination Reporting, Audit & Compliance Team Liaison & Patient Support Monitor and check tasks and work flows Keep directories and data bases up to date to ensure an efficient work flow Demonstrate duties to new or less experienced staff Resolve job related issues, including patient documentation and help resolve minor problems Photocopying Dealing with mail internal and external Attend meetings when required To work safely at all times in accordance with legislation and Practice policies and procedures Be flexible both in the availability and being able to cover for colleagues absent from work due to annual leave or sickness Any other reasonable duties requested by the Practice Manager or GP

In addition to the above, the post holder will show respect and courtesy to patients and colleagues, maintain confidentiality of patient and other Practice information at all times. Working occasional extra hours may be required.

All members of staff may be asked to attend periodic Staff Meetings, which may be held outside their normal working hours. They may also be required to work extra hours covering for holidays and sickness. This may include Saturday mornings and possibly weekend work in future on a rota basis.

Person Specification

Qualifications Essential

Proven experience of working in a GP Surgery Understanding of QOF and longterm condition management Working knowledge of medical terminology. Experience of handling confidential information. Proven experience of working with patients/public. Ability to handle telephone queries and understand when to involve others. Record and process messages accurately. Trained in the requirements of GDPR

Desirable

Care Coordinator training is preferred or willing to complete

Experience Essential

Knowledge of Emis Web and Clinical searches.

Desirable

Experience of working collaboratively with different organisations, building trust, confidence and partnerships

Motivation and Skills Essential
