Main duties of the job
The delivery of Community Multi-Disciplinary Team meetings (MDTs), contribute to the Integrated Neighbourhood Teams (INT) model and is one of the functions required to achieve Neighbourhood health in Sunderland. Multi-Disciplinary Team membership includes both health, social care and voluntary care sector providers from across the city, working together to identify patients most in need of a multi professional approach to interventions that help them stay well at home for longer.
The MDT Coordinator provides non-clinical administration and organisation support to facilitate this effective MDTs, working as part of a team to support 37 practices across the six PCNs of Sunderland. The MDT Coordinator provides supports to facilitate effective multi-disciplinary working across the system and to improve patient care.

Facilitation
Facilitation of close MDT working across all sectorsLeading on and coordinating practice-based and specialist MDT meetingsCo-chairing of MDT meetings across the city with clinical leadsEnsuring timeliness and focus of meetings, collation of patient names flagged for discussion, distribution of documents and action plans.Gathering relevant information to 'set the scene' prior to the MDTCompletion of all administration including liaising with Adult Social Care to determine if any current involvement and if appropriate seeking feedback from named worker.To be the first point of contact for GP Practices and other providers to guide and assist in maximising effectivenessProactively seek and recommend areas for improvement.Support General Practice with risk stratification and identification of new patients for discussion

Person Specification
Experience Essential
Substantial and appropriate experience of working in a busy administrative environment Facilitation of meetings Identifying and delivering continuous improvement Handling confidential information. Understanding of local health services and/or experience of working in an NHS setting or similar.
