Job summary
The Role offered is for 30 hours per week.The Social Prescriber will work with individuals to identify what matters most to them and co-produce personalised plans that support their health and wellbeing.The role focuses on addressing wider determinants of health, such as social isolation, housing, financial concerns, and lifestyle factors, by linking individuals to appropriate community resources and services.The postholder will work collaboratively with primary care, voluntary, and community sector partners to deliver holistic, non-clinical support.
Main duties of the job
You will provide personalised, one-to-one support to individuals referred into the service.Use coaching and motivational interviewing techniques to identify "what matters" to the person and co-produce simple, personalised care and support plans.You will support individuals to access appropriate community services, groups and activities and empower individuals to build confidence, independence and resilience.Work as part of the multi-disciplinary team within Primary CareBuild strong relationships with voluntary, community and statutory servicesAct as a bridge between health services and the CommunityIdentify and respond appropriately to safeguarding concernsRecognise risk and escalate issues in line with local policiesMaintain professional boundaries and confidentiality at all times

Main duties of the role
Taking and actioning referrals from GPs, Nurses and other practice staff as required.
As part of the PCN Multi-disciplinary team, build relationships with staff in the PCN Practices, attending relevant MDT meetings.
Be proactive in developing strong links with all local agencies to encourage referrals.
Support the PCN requirement to deliver Enhanced Access, delivering social prescribing during the out of hours sessions if the need arises.
Providing personalised support to individuals, their families and carers to take control of their health and wellbeing and to improve their own health outcomes.
Connecting people to appropriate local community groups and statutory services as necessary.
Managing and prioritising your own cases with your colleague. It is vital that you have a strong awareness and understanding of when it is appropriate or necessary to refer back to other health professions/agencies when their needs are beyond the scope of the SPLW i.e. when there is a mental health need requiring a qualified practitioner.
Informing and guiding non-clinical and clinical staff within the Network on what services are available within the community and how and when patients can access them
Person Specification
Experience Essential
Valid UK Driving Licence is essential for the role as you will be working across the GP Practices. Experience in the following: Supporting people with mental health either in a paid, unpaid or informal capacity. Working with VCSE sector including with volunteers and small community groups. Partnership/collaborative working and of building relationships across a variety of organisations. Working as proactive member of a multi-disciplinary team. Experience of supporting people, their families and carers in a related role including unpaid work.
Desirable
Experience of working directly in a community development context, adult health and social care, learning support or public health/health improvement including unpaid work. Experience of working as Social Prescriber. Training in motivational coaching and interviewing or equivalent experience. Experience of working in Primary Care
Qualifications Essential
NVQ Level 3 or equivalent. Experience of working with individuals in a support, health, social care or community setting. Strong communication and interpersonal skills. Ability to build rapport and trust with a wide range of people. Knowledge of community services and/pr willingness to lean quickly. Ability to work independently and as part of a team. Good organisational and time management skills. Basic IT skills (eg Microsoft and practice clinical systems).
