Do you want to make a difference by supporting patients in Primary Care with your skills as a Social Prescriber. An exciting opportunity has arisen to recruit a SPLW to joining our already established service, the applicant should be an enthusiastic and empathetic individual, to work in Northolt PCN - we comprise of 6 practices, working together to provide the best possible care to our patient population - We are an innovative, supportive and friendly team and look forward to having a Social Prescribing Link Worker join us.
Main duties of the job
Social prescribing empowers people to take control of their health and wellbeing. Non-medical link workers give time to individuals and focus on what matters to me they take an holistic approach, connecting people to community groups and statutory services for practical and emotional support. Link workers support existing groups to be accessible and sustainable and help people to start new community groups, working collaboratively with all local partners. The Social Prescriber post holder will be working in partnership with the existing SPLW and the practice teams in their Primary Care Network (PCN) to support patients. The Social Prescriber will have a good understanding of social prescribing, well-being and community services together with a good understanding of and ability to provide motivational and behavioral techniques.
About us
Northolt Primary Care Network started in the year 2019. We have 6 GP practices in the PCN. You will be working with 1 or more practices within the PCN.
You will be reporting to the PCN clinical Director and the PCN Board. Your Day to day clinical supervision is received from the Partner or their Nominated Deputy in the practice in which you are working or attach to.
We aim to Provide high quality accessible primary medical care. To follow up to date guidelines for best clinical practice. To ensure and support continued professional development of the Primary Health Care Team To provide an open, safe and pleasant environment for staff and patients alike To treat all patients in a polite, fair and equitable manner. Job description Job responsibilities
Social prescribing can help to strengthen community resilience and personal resilience, and reduces health inequalities by addressing the wider determinants of health, such as debt, poor housing and physical inactivity, by increasing peoples active involvement with their local communities. It particularly works for people with long-term conditions (including support for mental health), for people who are lonely or isolated, or have complex social needs which affect their wellbeing. Car Driver is essential for this role as you will be travelling across the PCN geography on a regular basis
Person Specification
Skills and Knowledge Essential
Demonstrable experience of working directly in a community development context, adult health and social care, learning support, public support or public health/health improvement. Experience of working with the general public. Experience of working within the NHS/Social Care. Experience of working within a multi-disciplinary team. Experience of working under pressure and managing varying workloads and changing demands. Excellent communication skills with the ability to communicate orally and in written form in a confident, articulate manner. Ability to build relationships with others to enable and support collaborative working. Relevant experience of General Practice IT systems
Experience Essential
Experience of supporting people, their families and carers in a related role (including unpaid work) Experience of partnership/collaborative working and of building relationships across a variety of organisations. Experience of data collection and providing monitoring information to assess the impact of services Full driving license and access to a car. Ability to travel across the locality on a regular basis, including to visit people in their own homes
Qualifications Essential
.GCSE grade A* to C (9-4) in English and Maths or equivalent .Demonstrable commitment to professional and personal development .Willingness to work towards further educational qualifications required for the role
