Job summary

Are you an enthusiastic and dedicated Advanced Clinical Practitioner and a registered Nurse or AHP or Pharmacist?

An exciting opportunity has arisen for an Advanced Clinical Practitioner to join our Sefton Acute Visiting Service (AVS) on a bank basis.

The AVS responds to both own home and care homes. Our service has two elements, a same day GP booked acute visiting service and a two hour community response. AVSoperates Monday to Friday between 09.00 and 18.00 and our community response service operates Monday to Sunday between 08.00 and 20.00.

The successful candidate will hold an MSc in Advanced Clinical Practice or Credentialing qualification and a Non-Medical Prescribing qualification. You will be able to workwithin the four pillars of practice. As an Advanced Clinical Practitioner you will act as a positive role model supporting junior staff whist working flexibly between urgent care and primary care, where appropriate and within a job plan, to provide safe, clinical and expert care.

Main duties of the job

Consult via telephone, video or face to face, according to patient needs. (Full training and support for remote consultations will be available if required).Supported by the non clinical team who have a good knowledge of our extensive local hospital and community services.Possess leadership qualities and provide support to the wider team, act as a positive role model and proactively identify areas for quality improvement. You will proactively contribute to improving health outcomes for patients by identifying new initiatives / ways of working and as such promote health improvement and disease prevention.Assist to further improve standards of care and, achieve QoF targets as well as KLOE objectives, as outlined by CQC.Have full access to our gtd learning academy and clinical forums, including the NMP monthly forum, which is available to all our staff. Clinical Practice

Assess and holistically clinically examine patients with both minor and complex needs through history taking, physical examination and appropriate use of investigations to confirm diagnosis.Plan, implement and evaluate an effective management plan for the patient, ensuring safe discharge/referral and/or transfer to an appropriate health care service.Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions.Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.Refer and discharge safely ensuring that safety-netting advice is offered.Triage telephone referrals, calls from external clinicians, patients and others into a range of services appropriately as required.

Communication

Utilise and demonstrate sensitive communication styles, to ensure patients are fully informed and consent to treatment.Communicate effectively with patients and carers, recognising the need for alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating.Anticipate barriers to communication and take action to improve communication.Maintain effective communication within the organisation and with external stakeholders.Ensure awareness of sources of support and guidance and provide information in an acceptable format to all patients, recognising any difficulties and referring where appropriate.

Non-Medical Prescribing

If you are employed in a prescribing capacity, you must only prescribe within your scope of practice and comply with the gtd healthcare NMP governance registration processes.You must maintain your knowledge and skills and prescribe within your scope of clinical competencies.The non-medical prescriber is required to comply with organisational NMP governance processes and adhere to local and national prescribing guidance.

Mentor/Teaching qualification. Clinical supervision training and experience.

Experience Essential

Experience of working as senior clinician. Experience of autonomous practice. Experience of leading/managing quality improvement. Experience of having worked in Primary and Urgent Care.
