Job summary Package Description: We are seeking an experienced and motivated District Nurse Team Lead to join our community services, providing strong clinical and operational leadership within a busy and rewarding environment. You will work as part of the 24-hour district nursing service, under the direction of the district nursing Team Manager / Leader, providing high quality holistic care to patients. This is a full-time role working 37.5 hours per week based at The Hollies BA3 2DP. The ideal candidate will have -

Essential:

Registered General Nurse Diploma in health-related area or equivalent experience Experience working as part of a multi-disciplinary team/multi agency working You need to have knowledge and experience of undertaking complex assessments and the planning and evaluation of care package Excellent communications skills both written and verbal Evidence of effective interpersonal skills Clinical Risk assessment skills Ability to problem solve and make clinical decisions Knowledge of safeguarding adults and at-risk vulnerable adults Excellent computer skills, including the use of electronic clinical systemsDesirable:

Teaching/mentorship post-registration qualification Non-medical prescriber Clinical audit Experience in Palliative Care Understanding of clinical governance framework and implications for practice Knowledge of lone working risks and associated risk management Motivated to self-developPackage Description

Person Specification

Qualifications Essential

Essential: Registered General Nurse, Diploma in health-related area or equivalent experience, experience working as part of a multi-disciplinary team, knowledge and experience of undertaking complex assessments, excellent communications skills both written and verbal, clinical risk assessment skills, knowledge of safeguarding adults. Desirable: Teaching/mentorship post-registration qualification, Non-medical prescriber, Experience in Palliative Care, Understanding of clinical governance framework.
