Staff Nurse (Primary Care Mental Health Practitioner)
We are seeking to recruit a Staff Nurse to join our team
Our transformation work aims for teams of clinical and non-clinical staff to work together to make a greater positive impact on the health and lives of groups and communities of people living with mentalill-health. Through providing more integrated care across mental and physical health services in innerLondon, we have been able to test new ways of working within our 'blended' teams, to deliverimprovements aligned to the implementation of the Community Mental Health Framework for Adults and Older Adults.
A key feature of our approach has been the development of a care offer delivered by staff from multiple partner organisations within the footprint of a Primary Care Network to deliver what really matters to people who need mental health support in that neighbourhood.
This role (which is jointly funded by the Primary Care Network (PCN) and ELFT) will be embedded in this new team structure, and represents an exciting opportunity to accelerate the transformation work underway across East London, while also bringing additional capacity to the new care models for those with serious mental illness, complex emotional needs, and disordered eating. You will support local GP practices to help rebuild and reconnect healthcare teams
Main duties of the job
As a registered Nurse, you will be expected to work with some autonomy within this system, but
would also connect into professional support structures already in existence in ELFT. You would also
be expected to work closely with other roles as part of the PCN's core multi-disciplinary team to help
address the potential range of biopsychosocial needs of service users with mental health problems
via personalized approaches (for example, PCN clinical pharmacists and social prescribing link
workers). You will be responsible for patients allocated to you, and will work alongside them to
assess, plan and deliver their care, including being professionally and legally accountable.
About us
Our mission is to make a positive difference to people's lives by improving the quality of life for all we serve. Our values are: We Care, We Respect, We are Inclusive - so we are looking for people who live and breathe these qualities when supporting service users and carers, and in their relationships with colleagues in the Trust and our partner organisations.
