About this role
Job summary
We are seeking an enthusiastic and motivated Senior Physiotherapist to join our Integrated Prevention and Reablement Services within Wrexham Social Services. This varied and rewarding role offers the opportunity to make a real difference, supporting people to remain independent and live safely in their own homes.
You will work closely with hospital teams to support safe and timely discharges, while collaborating with social workers, occupational therapists, social care assessors and other health and social care professionals. You will undertake physiotherapy assessments in people's homes, taking a holistic approach to identify the support needed to meet individual goals and needs. This is an excellent opportunity for a physiotherapist who enjoys multidisciplinary working, values person-centred care and is passionate about promoting independence.
