To provide allocation of patient visits for support workers and clinician's within the multidisciplinary integrated team. To build patient schedules and keep patient records updated To be an effective member of the Multidisciplinary Team. Communicate effectively with staff patients and carers as required. To complete all administration duties associated with the role To carry out other duties under indirect supervision of a registered professional.

To work within MDT Discharge to Assess/intermediate Care at Home which delivers community based services to a patients within Stockport .The job includes regular telephone contacts with patients, relatives and colleagues. Demonstrate excellent communications skills with patients, relatives, and colleagues. Maintain patient records, and demonstrate principles of patient confidentiality in accordance with Caldicott guidelines.Team case load updated, ensure records maintained to reflect the current needs of the patient and documented accurately on the team handover. Keep a daily account of bed and capacity availability.

Job description Job responsibilities
Main Duties and Responsibilities:
To work within the multi-disciplinary Discharge to Assess/intermediate Care at Home which delivers community based services to a patients within the Stockport area,
To undertake tasks which involve the management and maintenance of the teams clinical caseload, including all associated administration tasks.
The job will include using a variety of computer based programmes.
The job will include regular telephone contacts with patients, relatives and colleagues.
To undertake some responsibility for the team rota to ensure a professional staff mix is available to meet the needs generated by the clinical case load.
To be responsible for data collection, based on the outcomes from the clinical caseload.
To participate in and organise team audits when appropriate.
Communications
Demonstrate excellent communications skills with patients, relatives, and colleagues.
Have an understanding of support services available in the community.
Maintain excellent patient records, and demonstrate the principles of patient confidentiality in accordance with Caldicott guidelines.
Recognise and respond to challenging behaviour.
To maintain close liaison with colleagues and to communicate appropriately with other departments/services as required.
Pass messages to colleagues in a concise and timely way.
Responsibility for Patient Care
Ensure that the team case load is kept up to date.
Ensure that records are maintained to reflect the current needs of the patient and documented accurately on the team handover.
To keep a daily account of bed and capacity availability in the team.
Ensure patient related is equipment is fit for purpose and available for use be team members.
Participate in Health Promotion programmes with patients and carers, encouraging self-awareness and self-help.
To assist in the support and education of family/carers.

If successful this vacancy will require a Standard DBS check at a cost of £26.90. This cost will be deducted from your first Trust salary
In addition to this it will be a condition of your employment that you will join the DBS update service and pay the annual subscription fee of £16

Qualifications Essential
Maths and English - GCSE Level 3 or above Experience / patient customer service IT Skills including Excel, Microsoft packages and Word

Care Certificate Information technology qualification NVQ level 3 or equivalent in Health and Social care Evidence of other educational achievements
