Job summary

This is an exciting opportunity for a motivated and dedicated practitioner to work within a service that is embracing positive change and developing pathways of care to meet the diverse needs of older people within an inpatient setting.

The team comprises of a Consultant Psychiatrist, Occupational Therapist and Physiotherapist. You will be working alongside a dedicated team of nursing staff including Advanced Clinical Practitioner, Clinical team leaders, staff nurses and health care assistants.

The post holder will be supported closely by the Ward Manager and Lead Nurse and the Matron. Recognising the national pressures within acute inpatient services, South West Yorkshire Partnership Trust have developed a priority inpatient program focussing on the development of our inpatient services with priority focus on workforce, clinical pathways, and management of clinical acuity.

The Poplars is a mental health unit for individuals with either a diagnosis or presentation of symptoms of dementia. The Poplars predominantly supports individuals who are over 65 years of age, although on occasion there have been admissions of those who are under 65 years old. It is a 12 bedded mixed gender ward. The Poplars is a service that runs 24 hours a day, 7 days a week.

Main duties of the job

We are looking for an innovative, committed, and caring practitioner. The successful candidate will be required to work autonomously and as part of the MDT, demonstrating sound clinical decision making with a passion for working in within in-patient older peoples service.

As a Staff Nurse on a busy inpatient unit, you will support the MDT in the provision of high-quality care for older people with a diagnosis of dementia. You will be confident, have good people skills and able to challenge tactfully and constructively and able to gain the respect and confidence of nursing, medical and MDT staff. The successful candidate will be expected to work a 24hr shift system so to ensure care is provided across the day and night. You will also be supported to gain invaluable experience and grow in confidence Applicants are encouraged to contact the ward for further information.

To contribute to an integrated team of staff in providing a high quality of care and treatment both on an individual basis and with a purposeful group programme within a core facility.

Be responsible and accountable for the Assessment, Planning, Implementation and Evaluation of treatment and care for defined service users within the pathway.

Utilise core nursing / specialist skills within the role of primary nurse.

Contribute specific professional support and advice to the team, service users and carers.

Work in partnership with other teams within and outside of the organisation.

Oversee and support junior members of staff in implementing planned treatment and care.

Support the Management Team in carrying out their duties.

Workload Management

To work flexibly to cover shifts to provide a 365 day per year service Act as shift co-ordinator as required and to participate in the planning and co-ordinating of the shift to ensure smooth and effective running of the service and role model best practice Co-ordinate and lead the team to make effective use of team skills to provide the best possible standards of care and treatment / interventions To prioritise workload in keeping with the team and service objectives Take full involvement as a primary nurse by assessing, planning, implementing and evaluating treatment / care for named service users To actively participate and contribute to service users reviews, ward rounds, meetings as applicable To provide care and specific interventions for other service users as allocated on a daily basis, and directly in accordance with their written care plan

Person Specification

Qualifications Essential

1st Level Registered Nurse on the appropriate part of the register

Desirable

Relevant post basic qualifications CBT/PSI. Experience of mentoring/supporting learners or ability to demonstrate the skills required for effective mentoring.

Physical Attributes Essential

Ability to undertake the duties and demands of the post, working various shift patterns including nights. A satisfactory sickness record over the previous 2 years (subject to the need to act with fairness and equality of opportunity, particularly where the sickness is related to a disability and/or pregnancy).

Personal Attributes Essential
