Job summary

22.5 hours per week

The post holder provide will work within the Trust Wide Bladder and Bowel(Continence) Service to act as a specialist resource, facilitating,collaboratingand working in partnership with patients,carers and all relevant professionals to provide a service that best addresses patients continence needs.

The Senior Continence Advisor( HCA)will jointly supervise and support unqualified members of theBladder and Bowel( continence)service asappropriate alongside qualified team members.

Main duties of the job

Work within of The AdultBladderand Bowel (Continence)Team and contribute to a high standard of clinical care within trust policies to enable patients to receive the correct products and/or support to meetthereneeds.

To work on own initiative and within scope of competence, referring appropriately to the Clinical Nurse Specialist.

Along sidethe Adult Clinical Nurse Specialists provide specialist continence care to individuals contributing to triage, holistic assessment and management of care

Contribute to the development, implementation and evaluation individualised interventions using evidenced based advice and information to provide the right care in the right place at the right time.

Provide training and education to other professionals and carers asdeemedappropriate regardingcontinence care.

Maintaining excellent communication within the team providing relevant supervision to administration staff where necessary to deliver a high standard of care. Lead andfacilitateclinical supervision groups to junior staff within the service.

Work in partnership with other healthcare professionals and multi-agency workers, attending relevant multi-agency and other meetings asrequired.

Provideinformation to patients, their families, and carers both formally and informally, so that they can make informed choices about current and future care needs.

Person Specification

Education/Qualification/Training Essential

English and Maths (A*-C or 9-4) GCSE or equivalent qualification. Care Certificate, an equivalent qualification or equivalent experience of delivering compassionate, safe and high-quality care.

Previous Experience Essential

Demonstrable experience of delivering personal care and a range of related procedures, some of which may be non- routine, using safe evidence-based techniques and maintaining dignity and respect at all times. Demonstrable previous experience delivering delegated clinical care to patients/clients in a healthcare setting e.g. blood pressure, venepuncture, urinalysis etc. Experience of working in a care role in a community setting

Knowledge, Skills and Ability Essential

Knowledge of care and related procedures through experience or short courses Proficient IT skills; able to input, save and open information in a variety of systems. Ability to effectively communicate with colleagues in a variety of roles and seniorities, and patients where there may be barriers to understanding or engagement. Understanding of personal capabilities and limitations, with the ability to know when to escalate to senior colleagues. Demonstrate reflection to identify areas for learning and development. Ability to effectively prioritise activities to maximise efficiency.

Additional Requirements Essential

Ability to travel to multiple sites during the course of the working day where the location may be challenging to reach via public transport. Valid driving licence
