This is an exciting opportunity for an Assistant Practitioner to work as part of our multi-disciplinary Integrated Neighbourhood Teams, working within the community therapy team.
By joining us, you will have many opportunities to develop your clinical skills, physical assessment, and clinical reasoning, with the support of the wider team. You will work alongside Physiotherapists, Occupational Therapists, Community Support Workers, and the wider multidisciplinary team (MDT) to support people in facilitating safe hospital discharges and preventing admissions where appropriate and safe to do so. You will also work within our planned workstream to promote self-management and support individuals to maintain their independence within the community.
Main duties of the job
You will work as a member of the therapy team to support the organisation and facilitation of clinical care in a holistic and proactive manner to patients in our service.
You will:
Provide holistic assessment and treatment to patients on the caseload with the guidance of the wider team Maintain a high professional standard of care in adherence to all protocols/policies & guidelines Manage a caseload within your scope of practice and with the support of the wider team Provide clinical assistance to other members of the clinical team Maintain accurate electronic clinical records You will work within our service that is delivered from 8am-8pm, 7 days a week service.
You will be required to respond to referrals across urgent and planned pathways including hospital discharge facilitation.
You will work as part of the team to proactively monitor and manage patients on the caseload to ensure the best outcomes for them.

Person Specification
Qualifications Essential
Successful completion of a relevant Level 5 Foundation Degree OR an equivalent qualification, along with demonstrable knowledge and experience at degree level
Experience Essential
Experience of working in a health care or social care setting providing direct patient care Ability to communicate in varied ways with colleagues, service users and other stakeholders Demonstrate knowledge and understanding of Clinical Governance and Clinical Risk Able to evidence experience of working autonomously, and ability to know when to seek appropriate advice / guidance from senior staff Evidence of effective time management and caseload management
Desirable
Community work demonstrating a range of clinical experience Evidence of MDT working and clinical escalation
