Job summary
This role will suit a clinical Pharmacist looking to build on their clinical knowledge and develop their patient facing skills in supporting the patient journey from the ward to discharge into the community.
The post holder will be supporting inpatient Community Hospital wards at several sites in Dorset, based on their location. They will be ensuring seamless transfer of care back into primary care, supporting the discharge process and identifying patients in need of follow up, providing clinical advice to the staff, patients and our Pharmacy Technicians who provide follow up visits in the patients own homes.
As an independent prescriber, their will be opportunity to prescribe and deprescribe medication on the wards.
Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of. Applicants who subscribe to the DBS update service are able to present a valid DBS certificate instead of requiring a new check.

Main duties of the job
Main duties include:
Provide medicines management support in delivering Pharmacy Services to Wards and neighbourhood health hubsa medicines optimisation and clinical pharmacy service within community hospitals and neighbourhood health hubs promoting a seamless medicines service for transfer into patients homes.To support and where necessary participate in a service which receives patient referrals from external stakeholders for pharmacy patient support in the domiciliary setting that improves patient outcomes

Knowledge, skills and training Essential
Vocational masters degree in Pharmacy (4 years or equivalent) plus one year pre-registration training Registered with the General Pharmaceutical Council Broad clinical pharmacy knowledge Good Organisational Skills
Desirable
Postgraduate clinical pharmacy diploma or equivalent experience. A member of or working towards Faculty membership of the Royal Pharmaceutical Society Independent Prescriber Knowledge of NHS primary and secondary care working
