About this role

Job summary

We are seeking a motivated and patient-focused Clinical Pharmacist to join our friendly and forward-thinking GP practice at Storrsdale Medical Centre. This is an exciting opportunity to play a key role in improving patient outcomes, supporting medicines optimisation, and working collaboratively within a multidisciplinary primary care team.

You will work closely with GPs, nurses, and administrative teams to deliver high-quality, safe, and effective prescribing practices while supporting patients with long-term conditions and complex medication needs.

Main duties of the job

As a Clinical Pharmacist, you will:

Undertake structured medication reviews and medicines reconciliationSupport safe, cost-effective prescribing in line with local and national guidanceManage and monitor long-term conditions (e.g. hypertension, diabetes, asthma)Respond to medication queries from patients and staffWork with the practice team to improve prescribing systems and processesDeliver medicines optimisation and safety initiativesSupport repeat prescribing systems and reduce prescribing errorsProvide patient-facing clinics and telephone consultations where appropriateContribute to QOF, IIF, and enhanced services deliveryLiaise with community pharmacies, PCN colleagues, and secondary care

Job PurposeTo work as part of the general practice team to provide expert pharmaceutical advice and clinical support, improve prescribing quality, and ensure the safe and effective use of medicines.

Key ResponsibilitiesClinical ResponsibilitiesConduct clinical medication reviews for patients with complex needsManage prescribing for long-term conditionsIdentify and manage medication-related risksImplement deprescribing where appropriateProvide patient education and support adherencePrescribing & Medicines OptimisationEnsure prescribing follows NICE guidelines and local formulariesMonitor high-risk drugs and undertake safety auditsSupport cost-effective prescribing initiativesManage repeat prescription processesPatient-Facing ServicesRun clinics (face-to-face/telephone/online)Address medication queriesProvide advice on minor ailments and medication usageQuality Improvement & GovernanceContribute to audits and service improvement projectsSupport implementation of prescribing safety alertsAssist in achieving QOF and enhanced service targetsCollaborationWork with GPs, nurses, practice staff, and PCN pharmacistsLiaise with community pharmacies and hospital teamsParticipate in MDT meetings where requiredTraining & DevelopmentSupport and mentor practice staff on medicines-related issuesMaintain CPD and professional registration requirements

Person Specification

Skills and knowledgePatient-focused and compassionate Ability to work independently and as part of a team Strong organisational and problem-solving skills Essential

strong clinical knowledge and decision-making skills Knowledge of NICE guidelines and prescribing frameworks Excellent communication and consultation skills Ability to manage workload effectively and prioritise tasks Good IT skills (EMIS/SystmOne desirable)

Qualifications Essential

Qualifications Masters degree in Pharmacy (MPharm or equivalent) Registered with the General Pharmaceutical Council (GPhC)

Experience working as a pharmacist in primary care, community, or hospital setting Knowledge of medicines management and optimisation Experience of working within a multidisciplinary team
