Job summary
Provides high quality clinical care to patients, in line with the trust values and in accordance with professional regulations.
Accepts delegated responsibility from the Nurse in Charge, for the care of a specific group of patients. Works in line with trust policies, objectives, protocols, and local guidelines and in accordance with NMC regulations
Works as part of a team to provide high quality, evidence based clinical care to patients and their carers.
Communicates in accordance with trust values at all times with patients, carers and other staff members.
Undertakes opportunities to develop and expand their nursing skills, acting as a supervisor for Nurse Associates in training, Health Care Support Workers or Nursing students.
Maintains high standards of professional responsibility and accountability and have an awareness of the ethical and medico-legal aspects of nursing in line with NMC regulations and local guidance.
Main duties of the job
1. Work as part of a designated clinical team delivering care that focuses on the direct needs of the patients and their family.
2. Contribute to the planning of care delivery in association with the Registered Nurse.
3. Contribute to ongoing assessments recognising when it is necessary to refer to others for reassessment and action.
4. Demonstrate the ability to recognise the changing priorities in both physical and mental health and when to escalate to the appropriate professional for expert help and advice.
5. Recognises and effectively responds to emergency situations seeking support from the Registered Nurse in a timely and appropriate manner.
6. Implements and evaluates individual care / treatment plans.
7. Improve the quality of care by contributing to the continuous monitoring of patients care.
8. To accept delegated responsibility from the Registered Nurse to monitor individual plans of care to meet the immediate and on-going needs of patients.
9. Undertake a range of agreed clinical skills including venepuncture, routine ECG recording, measure and interpret blood glucose levels

Ensures all policies and protocols relating to infection control are implemented and monitored, taking appropriate and timely action in response to ensuring the prevention and control of infection. 12. To administer medication within the limitations of the role, in line with scope of practice in accordance with NMC Code and trust policy.
13. Demonstrates the ability to recognise the effects of medicines, allergies, side effects, drug sensitivity, contraindications, and adverse reactions.
Ensures safe practice in line with Health and Safety guidance, for example moving and handling and correct disposal of sharps. 15. To oversee the care delivered to their designated group of patients by healthcare support workers, nursing students and apprentice nursing associates.
16. To educate patients and families about illness and conditions under the direct/ indirect supervision of the Registered Nurse.
17. To maintain contemporaneous, accurate and evaluative patient records in line with NMC Code and Trust policy.
18. Understand the roles of a range of professionals and carers from other organisations and settings who may be participating in the care of a patient and their family.
19. To place Hillingdon CARES values at the centre of patient care and implement these values in practice.
Professional responsibilities:
1. Provides a holistic and safe approach to patient care.
2. Practices in accordance NMC code of conduct, having an awareness of professional responsibilities within the code.
3. Develop new skills and knowledge pertaining to clinical area by undertaking competencies and training, as advised by the Registered Nurse.
4. Communicates effectively within the Multidisciplinary team and with external agencies, where required.
5. Uses risk assessment and management to maintain a safe patient environment.
6. Contributes to the reporting of incidents, in line with trust policy.
7. Actively participates in continuing professional development.
8. Be familiar and work within scope of trust policies and procedures, ensuring they are understood by other members of staff within the team.
9. Demonstrates a good understanding of the Mental Capacity Act, Deprivation of Liberty and safeguarding concerns and processes, applying principles to everyday practice and seeking advice from Registered Nurse/other registered healthcare professionals as required.
10. Maintains strict confidentiality in relation to patient information, in accordance with the Data Protection Act and Caldicott principles.
