Job summary
We are seeking a dynamic and compassionate Tissue Viability Clinical Nurse Specialist to join our Team at Hillingdon Hospital. This is a senior clinical role at Band 7, leading the delivery of a high-quality, evidence-based Tissue Viability service across the Trust.
As a key specialist, you will provide expert assessment and management of complex and non-healing wounds, support frontline teams with professional advice, and contribute to Trust-wide initiatives to reduce pressure damage and improve patient outcomes. You will champion excellence in wound care, drive education and improvement initiatives, and act as a role model for clinical leadership, embodying our CARES values.
The role includes managing referrals, guiding policy and formulary development, supporting safeguarding processes, and collaborating with multidisciplinary and external partners to deliver integrated, patient-focused care. You will also lead and support audits, quality improvement, data reporting, and the implementation of best-practice systems to enhance patient safety.

Main duties of the job
The post holder is responsible for delivering a high-quality, evidence-based Tissue Viability service across the Trust, ensuring robust systems for monitoring, prevention and management of pressure ulcers and wounds. A key part of the role is to develop and implement strategies, tools and processes that support the reduction of harm, improve patient outcomes, and strengthen clinical practice. The post holder will lead the development and maintenance of comprehensive monitoring systems to track pressure ulcer incidence and prevalence, ensuring data is used effectively for improvement. They will provide a responsive referral service and specialist assessments for patients with complex or non-healing wounds, supporting frontline staff to deliver safe and effective care.
Education and workforce development form a major component of the role, involving the planning, delivery and evaluation of Trust-wide training programmes on all aspects of Tissue Viability. By integrating theory into practice through workplace-based learning, the post holder will help staff acquire new knowledge and skills, strengthen clinical decision-making, and maintain high standards of wound care across all wards and departments. They will remain updated on national and local developments in Tissue Viability, ensuring these are translated into clinical practice and communicated to relevant teams.

Job description Job responsibilities
The role requires strong leadership, modelling professionalism and the Trusts CARES values. The post holder will line manage Tissue Viability Nurses, supporting their development, setting clear objectives, and creating a positive culture of learning and improvement. They will lead Trustwide pressure ulcer reduction campaigns, working with senior nursing leaders to embed sustainable improvements. The post holder will facilitate change management, lead on local Tissue Viability projects, and provide expert guidance during After Action Reviews following hospitalacquired pressure ulcers.
A significant part of the role focuses on governance, quality assurance and performance. The post holder will collaborate with the Clinical Audit Department to design, coordinate and analyse Tissue Viability audits, producing SMART action plans and meaningful reports. They will maintain accurate data systems, clinical records and referral databases, and ensure policies such as the Pressure Ulcer Policy and wound care formulary are regularly reviewed and updated. The role includes participation in research, quality improvement initiatives, policy development and the introduction of quality systems. They will ensure appropriate incident reporting, investigation and documentation, contributing to continuous learning and safer care.
Effective communication and partnership working are central to the role. This includes maintaining strong communication channels with the Harm Free and Tissue Viability Nurse Head of Safeguarding Adults and senior nursing leaders, providing expert advice to multidisciplinary teams, and adapting communication styles to meet the needs of staff, patients and carers. The post holder will work collaboratively with Infection Control, Procurement, safeguarding teams and external partners to support an integrated approach to Tissue Viability. They will also ensure mental capacity assessments related to Tissue Viability care are appropriately documented.
Finally, the post holder is expected to exercise a costeffective approach in all aspects of service delivery, balancing quality care with responsible resource management.

To operate a responsive referral service for complex and non-healing wounds and pressure damage
To provide specialist assessment to patients who have complex or non-healing wounds of all aetiologies and to enable the provision of an effective, high quality, evidence based Tissue Viability service
Facilitate the implementation and sustainability of new Tissue Viability systems/processes at ward level.
Integrate Tissue Viability theory into practice through interactive workplace learning
