Job summary

Join a supportive multi-disciplinary team where your skills will have a real impact. In this role, you will manage referrals, deliver timely assessments, and coordinate care for a diverse caseload within a Community Mental Health Team.

You'll promote positive mental and physical wellbeing, supporting individuals to engage in their recovery and maintain independence. Working collaboratively with service users, carers, and partner agencies, you'll ensure care is person-centred, safe, and effective.

With a strong focus on quality and continuous improvement, you'll contribute to service development, uphold high clinical standards, and carry out comprehensive risk assessments and management plans.

Main duties of the job

Care Planning: Assess individual needs and develop, implement, and regularly review personalised care and support plans.

Care Coordination & Liaison: Act as the main point of contact between service users, families, GPs, and other health and social care professionals to ensure continuity of care.

Assessment & Risk Management: Carry out mental health and wellbeing assessments, including risk and crisis assessments, and respond appropriately.

Case Management: Maintain accurate, up-to-date records and case notes using relevant systems.

Signposting & Support: Connect individuals with appropriate community resources, including housing, employment, and support services.

Compliance & Governance: Work in line with mental health legislation, policies, and best practice standards.
